REQUIRED SERVICES LIST - ITEMIZED ITEM # DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT 0001 Radiology Annual Compliance Testing for the following: Radiographic/Fluoroscopic Tubes (25) AEC Devices (25) Dental Radiographic Tubes (22) Computed Tomography Systems (4) SPECT Systems (3) SPECT/CT System (2) MRI Systems (2) In accordance with Statement of Work requirements; See SOW Section 1, Items 1.1, 8, and Table A. Required reporting and documentation costs shall be included in cost of line item. See SOW Section 1, Items 11 & 12 and Sections 5, 6, 7, 10 & 17 1 YR Annually 0002 Annual Compliance Testing for the following: 5 Dual Head SPECT Systems In accordance with Statement of Work requirements; See SOW Section 1, Item 3. Required reporting and documentation costs shall be included in cost of line item. See SOW Section 1, Items 11 & 12 and Sections 5, 6, 7, 10 & 17 1 YR Annually 0003 Quarterly Calibrations (Oct-Dec, Jan-Mar, Apr- Jun, Jul-Sep) for the following: -Dose Calibrators -Well Counters -Uptake Unit -Survey Meters In accordance with Statement of Work requirements; See SOW Section 1, Item 4.
