SOURCES SOUGHT NOTICE Community Nursing Home Services Indefinite Delivery Contract (IDC) Overton Brooks VA Medical Center Shreveport, Louisiana DESCRIPTION: This is a sources sought to determine the availability of potential sources having the skills and capabilities necessary to provide the appropriate level of Community Nursing Home (CNH) care to the beneficiaries at the Department of Veterans Affairs, Overton Brooks VA Medical Center, Shreveport, Louisiana located at 510 E. Stoner Ave, Shreveport, Louisiana. It is the intent of the Government to issue a competitive multiple award Indefinite Delivery Contract (IDC) type solicitation after all market research has been completed. The requirement is for Nursing Home Services in the following states catchment areas: Louisiana: Caddo, Bossier, Webster, Claiborne, Union, Lincoln, Morehouse, West Carroll, East Carroll, Madison, Richland, Franklin, Ouachita, Jackson, Caldwell, Winn, Jackson, Red River, Sabine, Desoto, Bienville, Caldwell, and Natchitoches parishes. Texas: Harrison, Marion, Panola, Cass, Gregg, Upshur, Rusk, Shelby, Bowie, Nacogdoches, Morris, San Augustine, Smith, Camp, Sabine, Red River, Titus, Red River, Angelina, Jasper, and Newton counties. Arkansas: Columbia, Union, Hempstead, Clark, Miller, Little River, Ouachita, Nevada, Howard, Sevier, and Ashley counties. The estimated period of performance will be from the Award Date for one (1) base year with four (4) option years to renew and shall be exercised at the sole discretion of the Government. The number of veterans requiring Community Nursing Home services (bed days) over the next five years will fluctuate and cannot be adequately determined at this time. Therefore, the award of a firm-fixed price Indefinite Delivery Contract (IDC) is anticipated. Please be aware all Federal, State and local labor laws will be applicable to this procurement including, federal minimum wage as provided in Section 6(a)(1) of the Fair Labor Standards Act. REQUIRED: Interested agencies will be Medicaid/Medicare as applicable to the location as a Certified Nursing Home Facility, licensed as a Nursing Home by the State(s) of Louisiana, Texas, and /or Arkansas in which its provider number is based, accreditation by a recognized accrediting body i.e. Joint Commission (JC) or Community Health Accreditation Program (CHAP). All interested vendors are invited to provide information to contribute to this market survey/sources sought including commercial market information. THIS IS NOT A SOLICITATION ANNOUNCEMENT. The purpose of this notice is to gain knowledge, in this order, potential qualified sources and their size classifications (Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hub zone, 8(a), small, small disadvantaged, woman owned small business, FSS/GSA contract schedule holders or large business) relative to NAICS 623110 Nursing Care Facilities (size standard $27.5 M). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this source sought synopsis, a solicitation announcement may be published on the FBO website. Responses to this source sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this source sought announcement. REQUESTED INFORMATION: (1) STATEMENT OF CABABILITY: Submit a brief description, three (3) pages or less, that demonstrates how your company can provide the requested services at time of award. Include your company s past experience in performing these services to the VA, other Government (Federal or State) agency, or for a private facility. (2) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small as defined by the NAICS (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. (h) In accordance with FAR 52.204-7, System for Award Management, prospective contractors must be registered in System for Award Management (SAM) and in VetBiz database if applicable (VetBiz registration is required for Service-Disabled Veteran Owned Small Businesses or Veteran Owned Small Businesses) under the applicable NAICS. Contractors may obtain SAM information at https://www.beta.sam.gov and Vet Biz information at http://www.vip.vetbiz.gov. To be eligible for award, contractors must be registered in the System for Award Management database. Registration is free and potential offerors are encouraged to visit the SAM website as no IDCs can be awarded to any offeror not registered in SAM.
