Oxford Immunotec T-Spot IGRA Testing for tuberculosis laboratory testing services for Southeast Louisiana VA Health Care System NAICS Code: 621511 Medical Laboratories Notice Type: Sources Sought This is a request for information (RFI) and interested sources only; it is not a request for quotations or offers. VA is conducting market research to identify potential sources for the Oxford Immunotec T-Spot IGRA tuberculosis laboratory testing services requirement for the Southeast LA VA Health Care System located in New Orleans, LA. The Government does not intend to award a contract as a result of this sources sought notice or to otherwise pay for any information received in response to this source sought notice. Any response received will be treated for informational purposes only. See Attached DRAFT Statement of Work. If your organization has the capability to provide these services in accordance with the attached draft Statement of Work and is interested in this opportunity, please respond to Felicia Lovelady, Contract Specialist, via e-mail at Felicia.lovelady@va.gov. Please address the following in your responses: 1. Point of contact, DUNS Number, complete mailing address, telephone number, and email address. 2. Socioeconomic status, if applicable. 3. Size for the applicable NAICS code. 4. Can your laboratory currently self-perform the requirement as described in the Statement of Work? 5. Provide an equipment list evidencing that the necessary equipment to perform T-Spot testing is currently on-site. 6. Does your laboratory currently have an established contract with Oxford Immunotec USA, Inc. to obtain the T-Spot test kits and accessories? Is your laboratory currently authorized by Oxford Immunotec to perform the T-Spot tuberculosis testing in your laboratory? Provide your current contract with Oxford Immunotec evidencing authorization. 7. What is your laboratory s date of T-Spot tuberculosis test implementation, i.e., patient testing was initiated and patient test results were first released? Provide documentation. 8. Were T-Spot tuberculosis testing services implemented and were patient T-Spot tuberculosis testing results being reported at the time of your laboratory s last on-site accreditation inspection? Provide documentation. If not, is your laboratory currently licensed/accredited for the laboratory discipline of this testing activity (Diagnostic Immunology General (lab certification code 220; CPT code 86481)? Provide documentation. 9. Have the T-Spot tuberculosis testing services been fully validated in accordance with regulatory agency accreditation requirements? Please provide documentation. Have the T-Spot tuberculosis testing services been approved by the Laboratory medical director named on the CLIA license and/or accreditation certificate? Please provide documentation. 10. Provide the approval date of the Laboratory medical director named on the CLIA license and/or accreditation certificate. 11. Based on the volume listed in the draft statement of work, provide confirmation of current staff with the required credentials to meet the demands. In the alternative, if current staff is not sufficient to meet the demands, address how long it will take after award notification to obtain properly trained/credentialed staff. Failure to address the above requests in your response to this request for information will directly impact market research for this requirement.
