The VHA Network Contracting Office (NCO) 16 – Houston is seeking information on how an interested firm could support a requirement of providing Primary Care and Mental Health services for the Mt. Home, Community Based Outpatient Clinic (Capitated CBOC), Arkansas. Background The Central Arkansas Healthcare System’s, Mt. Home, Community Based Outpatient Clinic (Capitated CBOC) provides high quality Primary Care and Mental Health services (using the Patient Aligned Care Team or PACT model) to veterans and to operate a CBOC located within the city limits of Mountain Home, Baxter County, Arkansas. The potential contractor would provide health care staff, medical facilities, medical equipment, supplies, and all administrative functions sufficient to achieve the level of care in a manner consistent with Veterans Health Administration’s standards, and meet the requirement and guidelines set forth in the most recent edition of The Joint Commission accreditation manuals. Requested Information Interested & Capable Responses: NCO 16 is seeking responses from companies that are interested in this requirement and consider themselves to have the resources and capabilities necessary to provide these services. Please respond with your firm’s: 1. Business name, business type, DUNS number, CAGE Code, socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.), and person of contact (including telephone number & email address). 2. Capability statement and summary of previous experience providing Primary Care and Mental Health services for the Veterans Health Administration or other similar healthcare facilities. Responses Interested parties are requested to respond to this RFI with a brochure, capabilities statement, or any customary industry form of information. Such information should be in PDF format or in Microsoft Word for Office 2010 compatible format and are due no later than 3 February 2020, 4:00 PM CST. Responses shall be limited to 10 pages (front and back) and submitted via e-mail to Troy.Davis@va.gov. Proprietary information, if any, should be minimized and MUST BE CLEARLY MARKED.
