Statement of Work ManoScan Catheter EM Z *ALL ITEMS HEREIN THAT MENTION ARE ONLY BRAND NAME PROVIDED BY MEDTRONICS REQUIREMENT This requirement is for the purchase Emergency purchase of reusable Manoscan Catheter, at the KCVA Medical Center. Currently, there are no Manoscan catheters within the facility and unable to complete this service. DESCRIPTION OF THE ITEMS AND QUANTITIES: NAME BRAND MEDTRONICS Part Number Description Quantity 3890 MANOSCAN ESO Z CATH 1 DESCRIPTION OF SUPPLIES REQUIRED Catheters / Probes: MANOSCAN ESO Z CATHETER Advanced catheters capable of pressure sensing. Reusable solid state tactile pressure sensing technology Catheters offer 36 channels and provide 432 points of measurements to create pressure image from pharynx to stomach. 18 impedance channels to display bolus transition from pharynx to stomach. 96 3D channels to provide 3-D EGJ visualization. Small diameter (2.7mm) Provides physiological map with single positioning. Warranty: Manufacturer shall warrant and imply that the items delivered here under are merchantable, fit for use for the particular purpose and will be free from defects in material and workmanship for a period of one year from the date of delivery. Contractor will replace all parts found defective within the period with cost of replacement, including any shipping costs, to be borne by the Contractor for 1 year or 150 uses which ever one comes first. Under no circumstance will any equipment covered by the guarantee be returned without (a) advance written notice to the contractor or (b) obtaining shipping instructions from the Contractor. Installation Price of initial installation shall be included in the vendors bid/pricing. Shall not commence until all required product is received and until any unsatisfactory conditions have been OPERATING CONSTRAINTS The contractor will be responsible for all training for the end user. On-site work will be performed during normal working hours of 8:00 a.m. through 4:30 p.m. from Monday through Friday. Excludes weekends and holidays.
