Statement of Work Dental Laser Introduction The Harry S. Truman VA Medical, Dental Clinic has determined the need to purchase a new Dental Laser to provide more treatment options for the patients of the dental clinic. Description of Use: This solicitation uses a Brand Name or Equal Description of the product required. This permits prospective contractors to offer products other than those specifically referenced by name. All offers must work with existing equipment that has already been purchased and is currently in use at the station. Justification: The dental laser will be used by dentist to provide additional treatment options to dental patients. Without this piece of equipment, we will be unable to provide these additional treatment options for patients. Requirements for the system are as follows: Ultradent Dental Laser, quantity 1 Gemini EVO Laser, Mfr Part Number 9120 Gemini 7mm Tip-25pk Gemini 5mm Tip-25pk Gemini Safety Glasses Laser unit Activation Pedal wit 2 AA batteries Fiber Delivery System Pre-Initiated Disposable Tips (10) Bending tool included in the tip box DC Power Supply Protective Eye Ware DELIVERY Delivery accepted at Harry S. Truman VA Hospital, 800 Hospital Drive, Columbia, MO 65201 weekdays 0800 AM-4:00 PM, excluding any government holidays. The Authorization requirements do not apply, and that a Security Accreditation Package is not required.
