SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a RFI/sources sought only. The purpose of this sources sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 236220 Responses to this sources sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 15 Network Contracting Office, is seeking sources that can provide the following: PERFORMANCE WORK STATEMENT (PWS) VAMC Marion, IL 62959 GENERAL: This is a non-personnel services contract to provide material and services, Replacing the X-ray Shielding Lead Glass between the Cath Lab Control room and Cath Lab. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. Description of Services/Introduction: The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform the removal and replacement of all X-ray Shielding Lead Glass between the Cath Lab Control room and Cath Lab, as defined in this Performance Work Statement except for those items specified as Government furnished property and services. The Contractor shall perform to the standards in this contract. Objectives: Perform all necessary actions to remove and replace all glass between Cath Lab Control room and Cath Lab with 1/16 lead equivalent laminated blast resistant X-ray Shielding Lead Glass and provide the manufacturer s supporting documentation which indicates glass lead equivalence and blast resistance. Scope: Vendor to provide following items and services: Removal and disposal of all existing glass Install new laminated X-ray Shielding Lead Glass Perform any/all modifications to existing framework to support installation of new glass Provide manufacturers supporting documentation which indicates glass lead equivalence and blast resistance upon completion of work Place of Performance: The work to be performed under this contract will be performed at VAMC Marion, IL 62959, within the confines of building 1 Cath Lab Control room and Cath Lab. Special Qualifications: All work should be performed by qualified and/or certified individuals during removal and installation of laminated X-ray Shielding Lead Glass. If you do, state how and what is altered; assembled; modified? Does your company have an FSS contract with GSA, VA NAC, NASA SEWP, or any other federal contract, that can be utilized to procure the requirement listed above? If so, please provide the contract number. Please submit your capabilities that show clear, compelling, and convincing evidence that you can meet the requirement described in the attached PWS.
