Page 2 of 2 The Department of Veterans Affairs intends to solicit proposals from contractors in the Leavenworth, KS and Topeka, KS metropolitan areas to provide homeless veteran services through a Transitional Housing model of residential care at a facility located within 15 miles of the Leavenworth VA Medical Center and Topeka VA Medical Center as part of its Health Care for Homeless Veterans (HCHV) program. The Leavenworth VA Medical Center is located at 4101 S 4th St, Leavenworth, KS 66048. The Topeka VA Medical Center is located at 2200 SW Gage Blvd, Topeka, KS 66622 The VA anticipates issuing a solicitation for this requirement on or about July 1, 2020, with a closing date for proposals on or about August 1, 2020. The solicitation will be available for download from http://www.fbo.gov. Interested contractors will be responsible for downloading copies of solicitation (and any subsequent amendments) from that website, and no hard copies will be made available. The solicitation will provide full information to contractors about the proposal submission process. Prospective offerors are encouraged to register their interest in the solicitation on the FBO website so that they are notified of the issuance of the solicitation and any amendments. The goal of the Contracted Emergency Residential Services Healthcare for Homeless Veterans (HCHV) program is to provide housing and case management services to Veterans and facilitate their access to a broad range of medical, mental health, and rehabilitative services. The target population for this HCHV Contract Residential Care is homeless Veterans who have permanent housing goals. Eligibility will be determined by the VA, based on veteran status; meeting the McKinney-Vento Homeless Assistance Act definition of homelessness (see http://www.hudhre.info/hearth/ for additional information); and an assessment by the VA HCHV program resulting in a determination of serious medical, mental health, substance abuse or other serious psychosocial conditions or stressors making the Veteran highly vulnerable with a high likelihood of death due to homelessness. This will be an indefinite delivery/indefinite quantity contract in accordance with FAR 16.5. NAICS Code is 624221 with a small business size standard of $12M. The solicitation will be issued for full and open competition. All responsible sources may submit proposals which shall be considered by the Agency. All vendors must be registered in the System for Awards Management database (www.sam.gov) and have input Vet Biz information at http://www.vip.vetbiz.gov .
