REQUEST FOR INFORMATION (RFI)/SOURCES SOUGHT DISCLAIMER This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is an RFI/sources sought only. The purpose of this sources sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS Code: 332311 Size Standard: 750 Employees. Responses to this sources sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 15 Network Contracting Office, is seeking sources that can provide one (1) or both the following: Each to be delivered, installed, maintained, and subsequently removed at the Dwight D. Eisenhower VA Medical Center in Leavenworth, Kansas. 1) Six (6) month rental of a Mobile Surgical Unit (MSU). Purpose of the MSU is to provide continuity of surgical care to patients while the main facility s Operation Rooms (OR) are being renovated. The MSU will be used for Standard Level, Ambulatory Advanced, and Ambulatory Basic procedures. The MSU will be maintained to the standard required by all applicable laws, regulations, and policies. 2) Six (6) month rental of two (2) climate-controlled storage containers. One (1) storage container will be used to store OR equipment and furniture while the facility s main OR is being renovated. One (1) storage container will be used for OR supplies and equipment needed on a case-by-case basis by the surgical staff in the MSU. Both storage containers must be able to be secured when not in use by staff. Special Requirements/Concerns: Each of these rentals directly impact the ability for the facility to provide continuity of surgical services to patients. The delivery and installation schedule must be closely coordinated. Operational timelines may be stringent. This project requires a contractor familiar with the VHA and healthcare laws, regulations, and policies. The Department of Veteran Affairs intends to award a contract with a six (6) month period of performance, with an option to extend monthly for up-to a total of six (6) additional months. The estimated magnitude of the entire contract is between $500,000 and $1,000,000. RESPONSES If you are interested and capable of providing the sought supplies/services, please provide all requested information. Response to this notice should include company name, address, point of contact, size of business pursuant to the following: (1) a positive statement of your intention to submit a bid for this solicitation as a Prime Contractor. Must clearly identify if you are able to meet one (1) or both of the requirements above; and (2) date of HUBZone certification; and/or (3) documentation of VOSB or SDVOSB status; and/or (4) 8(a) certification; and/or (5) WOSB status; and (6) listing of experience in work similar in type and scope to include contract numbers, project titles, dollar amounts, points of contact with their phone numbers, and an explanation of the relevancy of the project; (7) If you are a HUBZone considering a joint venture that has not been formalized, you will need to specify the firm(s) you intend to partner with and how you plan to officially form the joint venture; (8) Must include your DUNS Number. Firms interested should provide the above documentation. All of the above must be submitted in sufficient detail for a decision to be made on the availability of interested 8(a), HUBZone, WOSB, or SD/VOSB concerns. Responses shall be limited to five (5) pages. Failure to submit all information requested may result in a contractor being considered "non-responsive" to this sources sought. Please submit your capabilities that show clear, compelling, and convincing evidence that you can meet the requirement(s).
