The origina... NOTICE: This is a second (3rd) modification to a previously-issued pre-solicitation notice. This modification to a previously-issued pre-solicitation notice is issued for further response. The original pre-solicitation notice was modified two (2) times prior to this action. This modification seeks additional information regarding firms, current business practices and areas of operations, and current capabilities of firms related to current ability to provide the services of this Government requirement. As previously provided, the Government (Veterans Integrated Service Network (VISN) 15, encompassing VA Medical Center locations (primary) in: 1. Southern Illinois, Missouri and Kansas, and with outlying veterans services coverage areas also in Southern Illinois and their coverage areas including portions of Southern Indiana and portions of Western Kentucky. 2. Most of Missouri, and with outlying coverage areas into Northwestern Arkansas. 3. Most of Kansas.) requires a contractor or contractors who can: Receive, store and inventory and deliver Government-owned Durable Medical Equipment (DME) items to veterans’ homes; Maintain a facility of sufficient size (traditionally estimated to be +/- 1,500 square feet of dedicated space for Government items), and within 35 miles (highway, not radius) of each VA Medical Center; Have and maintain Joint Commission (or equivalent accrediting entity) accreditation, certification and provide services in compliance with DME services standards; Deliver DME items to veterans, including associated installations, assemblies, training/education and periodic support to the veterans. Deliveries include scheduled and emergent situations; Pick-up DME items, repair DME items, and return to storage or dispose of items, update and communication inventory changes; Maintain sufficient personnel with necessary training, licenses, ability and certifications to perform all aspects of DME services required by the Government. The actual solicitation will provide more specific requirements. This posting requests information in the form of responses to the questions below. Please ensure your response includes answering the questions below and include supporting information for each answer. Other information is not helpful without these being answered and supported. 1. Is your firm/business/entity current Joint Commission (or similar accrediting entity) accredited now? Yes/No? If Yes - Accredited for DME services? Other accreditation? Current to what date? Grand Blvd.; 63106; City of St. Louis - Columbia, Missouri – 800 Hospital Drive; 65201; Boone County - Kansas City, Missouri – 4801 Linwood Blvd.; 64128; Jackson County - Leavenworth, Kansas – 4101 St. 4th St.; 66048; Leavenworth County - Topeka, Kansas – 2200 Gage Blvd.; 66622; Shawnee County - Wichita, Kansas – 5500 E. Kellogg Dr.; 67218; Sedgwick County If Yes - Have space available for Government DME now? Have ability to receive shipments from a variety of shippers, shipper vehicle types (i.e. vans, flatbed tractor/trailer, box trucks, etc.) now? Can you now segregate Government DME from other stored items and spaces in your facility? Do you now provide DME services as previously described? For who? How large is/are your current space(s)? If No - How would you acquire the facility and/or space for Government DME and receive, store and maintain inventory? If yes, and if not, please explain how you would meet the Government needs. 3. Do you now provide DME services as described above and elsewhere in related postings in similar scale, i.e. for multiple medical patients for multiple hospital or clinical systems? Please provide examples of current DME services provision. If yes, and if not, please explain how you would meet the Government needs. 4. The nature of the work required includes medical patient contact, requiring a Business Associate Agreement (BAA) (reference HIPAA 45 CFR 160.103). Does your firm have in place a Business Associate Agreement now? Under what name(s)? National or local BAA? If yes, and if not, please explain how you would meet the Government needs. 5. The nature of the work requires delivery and pick-up. Does your firm now have necessary fleet(s) of vehicles (e.g. delivery van) to deliver and pick-up DME to multiple recipients per day on a daily (Monday through Friday) and sometimes weekends or holidays? If yes, and if not, please explain how you would meet the Government needs. 6. The nature of the work requires physical efforts, such as assembly, installation, disassembly, repair work, lifting/loading and similar activities. Does your firm have the necessary personnel to perform this work now? If yes, and if not, please explain how you would meet the Government needs. 7. The nature of the work requires inventory management, maintenance of records of inventory on-hand, minimums and maximums of inventory and communication of the same with the VA Medical Centers. Does your firm have these capabilities now? If yes, and if not, please explain how you would meet the Government needs. Does your firm current perform this? 8. The nature of the work requires ability to repair DME items, including specialty items such as electric hospital beds and electric wheelchairs and other items, requiring manufacturer-specific parts, methods, training, etc. Does your firm have these capabilities now? Does your firm have the capability of maintaining on-hand parts, supplies and materials to perform repairs? If yes, and if not, please explain how you would meet the Government needs. 9. The nature of the work requires disposal of DME items, some may require compliance with federal, state or local laws. Does your firm now have this capability? If yes, and if not, please explain how you would meet the Government needs.
