ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS COMMERCIAL ITEMS Provisions that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. The version of FAR 52.212-1 in the addendum is tailored for Simplified Acquisition Procedures and supersedes the current version of FAR 52.212-1 contained in the FAR. The following provisions are incorporated into 52.212-1 as an addendum to this solicitation: Submittal of Quote: Offerors shall submit quote by email to Lori.Eastmead@va.gov by the solicitation due date. Please limit graphics or excessive large files during quote submission. *VA has a maximum allowable incoming email size limitation including attachments of seven megabytes. Offerors submitting responses via email which exceed seven megabytes shall split their response into multiple email messages to not exceed the maximum allowable email size limitation. The VA is not responsible for late responses due to undelivered e-mails that exceed seven megabytes. CONTENTS OF QUOTE: The Government, in its sole discretion, will award a contract to the responsible contractor whose quotation, conforming to the solicitation, is most advantageous to the Government, considering all of the following factors (Note: The order of the factors listed does not imply the relative importance of each factor.): (1) Technical Capability; (2) Prior Experience; (3) Administrative Information; (4) Price; and (5) Representations & Certifications. The Government will evaluate quotations using the comparative evaluation process outlined in FAR 13.106-2(b)(3), where quotations will be compared to one another to determine which provides the best benefit to the Government based on the factors listed above. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation, even if that quote is not the lowest priced quote submitted but is not required to do so. Each response must meet the minimum requirements of the solicitation. The Government is not requesting or accepting alternate quotations. Quotes shall include the following information to be evaluated for contract award. Incomplete quotes will be considered non-conforming and may not be considered, pursuant to FAR 13.106-2(b)(3). Technical Capability: Offers shall submit training and certification of fire officers, engineers and firefighters as required by the State of Illinois and applicable NFPA Standards for municipal fire departments. Include all Federal, State and Local Licenses (If applicable). Provide documentation demonstrating required availability of apparatus per the Statement of Work (paragraph 1.4). Provide proof of established mutual aid agreement with at least two (2) other municipal fire departments. Provide documentation of the on-going comprehensive training program of all fire department personnel, to include proof that personnel meet Illinois state requirements for the held positions. Prior Experience Provide the number of years your company has been in this business. Have you worked on a government contract before? Have you ever received a termination under a government contract? If so, describe the circumstances of the termination. Provide a reference to include the facility name, location, contact person who is responsible for the administration of the contract, email address and the telephone number for each reference. Administrative Information: Complete PRICE/COST SCHEDULE Fill in SF1449 blocks 17a, 30a, 30b, and 30c on Page 1. Complete Acknowledgement of Amendments (if any), on Page 2. Complete subsection 1a, Contractor information, under B.1. Provide the documentation below via email to Lori.Eastmead@va.gov. Price: Complete the highlighted blanks in SECTION B.2 PRICE/COST SCHEDULE Representations & Certifications: The Government will give preference to offers received from VOSBs and additional preference to offers received from SDVOSBs. Preference shall be applied when all other factors (technical capability, prior experience, administrative information and experience, and representations & certifications) are equal. Technical Proposal shall also include the following documentation:            a.  Certificate of Insurance               b.  Appropriate Federal, State and Local Licenses (If applicable)            c.  Contractor s Employees Certifications and Training Certificates (Spreadsheet with corresponding training is acceptable) d.  Registration in SAM in accordance with FAR 52.204-7 System for Award Management (JUL 2013) NOTE: If Offeror is seeking SDVOSB/VOSB preference they must submit proof in accordance with 852.215-70, Service-Disable Veteran-Owned and Veteran-Owned Small Business Evaluation Factors and offeror must complete the certifications found at VAAR Clause 852.219-75 VA Notice of Limitations on Subcontracting Certificate of Compliance for Services and Construction found on page 30. The Government will not consider offers from offerors that do not provide the certification, and all such responses will be deemed ineligible for evaluation and award. SDVOSB joint ventures are eligible for consideration so long as the requirements of VAAR 819.7003(c) are met and are reminded to submit their socioeconomic status as provided in FAR 52.212-3(c) and FAR 52.219-1(c), in accordance with the authority of FAR 52.204-8(d) and FAR 52.212-3(b). Offerors who fail to submit required information and documentation will not be eligible for award.
