Contractor shall furnish all parts, labor, supervision, travel expenses, tools, equipment, and expertise necessary to provide preventative maintenance and corrective repairs on the Alcon LenSx Cataract Laser in strict accordance with all schedules, specifications, terms, conditions and provisions of this contract. Equipment is located at the Clement J. Zablocki VA Medical Center in Milwaukee, WI and the Jesse Brown VA Medical Center in Chicago, IL. NOTE: Locations of Service that shall be allowed to order off this contract should the facility purchase a machine at a later date: Captain James A. Lovell Federal Health Care Center North Chicago, IL Oscar G. Johnson VA Medical Center Iron Mountain, MI Tomah VA Medical Center Tomah, WI Edward Hines, Jr. VA Hospital Hines, IL William S. Middleton Memorial Veterans Hospital Madison, WI VA Illiana Health Care System Danville, IL The contract is for a base year plus four ordering periods of coverage. The base period of performance shall be from December 1, 2022 through November 30, 2023.
