STATEMENT OF WORK (SOW) Siemens Vista Maintenance and Service 1. Scope of Procurement Contractor shall provide all personnel, equipment, supplies, transportation, tools, materials, and supervision to perform preventative maintenance and emergency repair services for Siemens Vista analyzers at the following facilities: Jesse Brown VA Medical Center Tomah VA Medical Center Adam Benjamin Jr. Community Based Outpatient Clinic (Crown Point) 2. Specific Tasks/Deliverables Preventative maintenance visits shall be performed at the manufacturer s recommended intervals and shall be scheduled during normal business hours (8:00 AM to 5:00 PM, Monday Friday). Appointments for preventative maintenance shall be scheduled with Government laboratory personnel. Contractor shall provide Top 1 (24 hours per day, 7 days per week) repair service for the following: Facility Instrument Serial Number Jesse Brown VA Medical Center Dimension Vista 500 DV330866 Jesse Brown VA Medical Center Dimension Vista 1500 DV311136 Tomah VA Medical Center Dimension Vista 500 DV330863 Tomah VA Medical Center Dimension Vista 500 DV330865 Contractor shall provide Top 3 (8:00 AM 9:00 PM, Monday Friday) repair service for the following: Facility Instrument Serial Number Adam Benjamin Jr. Outpatient Clinic Dimension Vista 500 DV330884 Contractor shall provide unlimited service visits, including labor and travel, and all necessary replacement parts. Technical assistance shall be available by telephone twenty-four (24) hours per day, seven (7) days per week to troubleshoot equipment malfunctions. The maximum call back time is 2 hours. Repair service response time shall be no more than 12 hours from time of notification for all locations. Each notification for an emergency repair service call shall be treated as a separate and new service call. An incident report shall be furnished to the Government upon completion of each repair or preventative maintenance service. 3. Period of Performance: November 1, 2022 April 30, 2023 4. Hours of Operation for Preventive Maintenance Normal hours of coverage are Monday through Friday from 8:00 am to 5:00 pm CST, excluding holidays. All service/repairs will be performed during normal hours of coverage unless requested or approved by COR or designee. Holidays include: New Years' Day Labor Day Martin Luther King Day Columbus Day Presidents' Day Veterans' Day Memorial Day Thanksgiving Day Juneteenth Christmas Day Independence Day And any other day specifically designated as a national holiday by the President of the United States.
