The Department of Veterans Affairs Great Lakes Acquisition Center (GLAC) in North Chicago Il intends to negotiate a sole source contract with Roto Rooter 824 PIONEER ST CHAMPAIGN, IL 61820 for Sewer Mapping throughout Bldg. 128 at the Illiana VA Healthcare Systems at 1900 East Main Street, Danville, Il 61832. This contract will be for a firm fixed price contract. This procurement is being conducted in accordance with FAR 6.302-1, Only One Responsible Source and no other supplies or services will satisfy the agency requirements. The contract will be firm fixed price and the NAICS code is 541360 This Notice of Intent is not a request for competitive proposals. No solicitation documents are available and telephone requests will not be honored. However, if a firm believes it can meet the requirements it must furnish information about its products, services and capabilities as well as references from other customers who are using these products and services provide service for the Sewer Mapping located at the Illiana VA Healthcare Systems at 1900 East Main Street, Danville, Il 61832 to the contracting officer NLT 11:00AM CST, 20 July 2022. Supporting evidence must be submitted in sufficient detail to demonstrate the ability to comply with the above requirements and attached Statement of Work, to include access to OEM parts for repairs. Responses received will be evaluated; however a determination by the Government to not compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the GLAC will proceed with the sole source negotiation with Roto Rooter STATEMENT OF WORK Scope of work: The vendor shall provide all labor, material, supplies, equipment, management and supervision to conduct the sewer mapping throughout Bldg. 128 at the Illiana VA Healthcare Systems at 1900 East Main Street, Danville, Il 61832 Services to be provided: 1. Video scope the drainage system under Bldg. 128 a. Scope the drainage system with camera, recording drainage system, defects/blockages, identifying locations of defects/blockages +/- 3 and providing isometric drawings of underground drainage system. b. If defects/blockages are found the position of the defect/blockage will be marked on isometric drawing provided by vendor within +/-3 . With the location of the defect/blockage identified for future repairs. c. Recording of the video obtained during investigation will be provided with the paperwork. 2. Any drain covers, caps, plugs, etc. removed to access the drain system will be replaced when finished at all access points. CERTIFICATION REQUIREMENTS Illinois plumbing license REPORTING REQUIREMENTS 1. Need to list all defects/blockages found and notate them on drawing submitted by vendor after inspections. 2. Information provided shall include blueprint and specific location of the defects +/-3 on drawing provided by vendor. 3. Recommendations for prompt repair of any identified defect/blockage and location of defect/blockage shall be provided +/- 3 . Time period Inspection shall commence as soon as practical after award 1. Vendor shall coordinate with the (COR) to schedule work prior to arriving at site. Mapping will be done in such as manner as to minimize disruption to ongoing patient activities.
