REFERENCE SOLICITATION 36C25222Q0050 AND AMEND TO ANSWER SUBMITTED QUESTIONS Please see new attachment :S02 Ammendment 36C25222Q0050 0001 Consultant Engagement to Advise in Recruitment and Retention of Unique Veteran Patients The Complete Statement of Work is Attached in the Solicitation for Jesse Brown VA Medical Center. Please Note: The engagement shall not exceed 6 months and may occur in overlapping fiscal years. Work by the consultant team shall primarily take place offsite with regular virtual and in person meetings at the medical center. It is acknowledged that at the beginning of the engagement, consultant team shall need to spend time at the medical center and at the 4 outpatient clinics Community Based Outpatient Clinics (CBOC’s) in order to accomplish necessary research needed to inform strategy. Please see the list of the four CBOC’s locations listed as follows: Lakeside, Auburn-Gresham (both clinics are located in the City of Chicago), south suburban Chicago Heights, and the Adam Benjamin Junior CBOC located in Crown Point, Indiana
