STATEMENT OF WORK Scope of Work: Semi-Annual Maintenance for Liquid Scintillation Counters: Contractor shall provide all labor, travel, materials, equipment and parts to provide semi-annual preventive maintenance and repairs on the TRI-CARB 3110TR Liquid Scintillation Counter (Research and Development) and the TRI-CARB 2910TR Liquid Scintillation Counter (Radiation Safety Office). Preventive Maintenance visit will also check the accuracy of the unit's operation, and to recalibrate as required. Specific Tasks and Deliverables: Contractor shall perform semi-annual preventive maintenance and repair services on Perkin Elmer TRI-CARB 3110TR Liquid Scintillation Counter (SN: SGTC33140389) and TRI-CARB 2910TR Liquid Scintillation Counter (SN: SGTC36130071). Services include, but are not limited to: The technician consults with lab personnel to determine whether there have been reported malfunctions, then performs required set of tests to verify that each unit is reading and reporting results accurately. Perform manufacturer recommended preventive maintenance. Recalibrate system, as required. Provide telephonic support, as required. Hours of Operation: Normal hours of coverage are Monday through Friday from 7:30 am to 4:30 pm CST, excluding holidays. All service/repairs will be performed during normal hours of coverage unless requested or approved by COR or designee. Holidays include: New Years' Day Labor Day Martin Luther King Day Columbus Day Presidents' Day Veterans' Day Memorial Day Thanksgiving Day Independence Day Christmas Day And any other day specifically designated as a national holiday by the President of the United States. Place of Performance: Jesse Brown VA Medical Center 820 S. Damen Ave Chicago, IL 60612-3728 Period of Performance: Base Year: August 1 through July 31, 2022 Option Year 1: August 1, 2022 through July 31, 2023 Option Year 2: August 1, 2023 through July 31, 2024 Option Year 3: August 1, 2024 through July 31, 2025 General: All work shall be performed by manufacturer trained technicians. All repair parts used shall be new and designed specifically for use on this equipment. No substitution parts or rebuilt/refurbished parts shall be used. Contractor shall schedule preventive maintenance services with the Contracting Officer Representative (COR). All parts, labor, travel costs and telephonic support are included in the contract price, and shall not be billed separately. Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. Security: Vendor will not have access to VA Sensitive Information, patient records, or data. For repairs or services performed during normal work hours, 7:00 a.m. to 4:40 p.m. at the Jesse Brown VA Medical Center, contractor s repairman shall sign-in and obtain a badge and return the badge at the end of the day. Repairmen will then report to the Contracting Officer Representative (COR) at the Research Department, Room 6215. Contractor personnel will be escorted while in the research department. After all work is complete, the contractor s repairman shall submit a written service report outlining the services performed and completed to include parts used.
