REQUEST FOR SF 330 – Amendment 0001 AE IDIQ for Wm. S. Middleton VA Hospital Madison, WI 36C252-21-Q-0102 The original request for SF 330’s is hereby amended as follows: Clarification on submission requirements: Is it expected that all of the Section F projects shown have an associated CPAR or PPQ printout included in the proposal? Not necessarily, the intent is to highlight the firm and its teams’ qualifications. IF CPARs / PPQs are expected to be included in the proposal, their page counts’ average ~3 each which could yield 30+ pages in CPARs / PPQs alone. Is it acceptable to the gov. to OMIT CPAR / PPQ printouts from page limitations? Please limit the CPARS submission to five projects. The CPARS will not count against the 75-page limit. Estimated Magnitude of Construction language is hereby changed to: A/E IDIQ Task Order Limits: Individual task orders will be established for a minimum of $3,000.00 and a maximum of $1,000,000.00. Contract Ceiling Language is hereby changed to: A/E IDIQ Contract Ceiling: $5,000,000.00 (This will be a single award A/E IDIQ) The ordering period for services under this IDIQ contract will be 5 years commencing on the date of contract award. The minimum amount of each task order will be $3,000.00 and maximum amount of each task order will be $1,000,000.00. The guaranteed minimum will be $10,000.00. The maximum amount of the contract will be $5,000,000.00. In the Solicitation, it only mentions scopes of work that may entail Civil, Structural, MEP and FP disciplines. Would you like us to provide additional subconsultants on this SF330, such as Environmental, Geotechnical, Commissioning, Physical Security, etc?? Or should we just provide the ones mentioned in the SF330, and at time of a possible contract signing for a specific Task Order, provide the additional Sub-Consultants then? We are not looking to overwhelm your office with additional information if it is not necessary at this time. Please follow the instructions within the Request for SF330. All other items of the original notice remain unchanged. There are no further changes.
