This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 2. Solicitation 36C25221Q0030 is issued as a request for quotation (RFQ). 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2019-02 4. Notice: Solicitation 36C25221Q0030 is a Total Service Disabled Veteran Owned Small Business Set-Aside, North American Industry Classification System Codes (NAICS) 621330, Small Business Size Standard $8 Million. 5. All line items are listed in the Cost and Pricing Schedule for base award and one (1) option year for cost per month, annual cost, and total contract value 6. Description of requirements: This is a non-personal service contract to provide counseling services for the Iron Mountain VA Medical Center and surrounding Community Based Outpatient Clinics. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. 7. Provision FAR 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition addenda for all items needed for submission are display in FAR 52.212-1 8. 52.212-2, Evaluation -- Commercial Items, applies to this acquisition. Quotes are to be submitted electronically and are due on October 26, 2020 by 9:00 AM Local Time. 11.
