B.3 PERFORMANCE WORK STATEMENT (PWS) 1. General Overview: Contractor shall furnish all labor and tools necessary to complete the inspection, testing, maintenance, and repair for the temperature control systems and devices specified in this Performance Work Statement (PWS) and elsewhere in the contract. All services shall be performed on-site at Jesse Brown VA Medical Center, Chicago, IL. Equipment is located throughout the facility in buildings: 1A, 1B, 5, 11A, 11B, 30, and 40. Services shall be performed in accordance with (IAW) the PWS; Conformance Standards; the original equipment manufacturers' (OEM) recommendations; any local, state, and federal standards; and VA Policies & Procedures; and The Joint Commission (TJC), including all emergency repairs. Performance Location: All services shall be performed on site at the Jesse Brown VA Medical Center, 820 S Damen Ave., Chicago IL 60610. The temperature control systems are located throughout the entire VA facility in buildings: 1A, 1B, 5, 11A, 11B, 30, and 40. Period of Performance: Base Year: 1 December 2019 30 November 2020 Option Year One: 1 December 2020 30 November 2021 Option year Two: 1 December 2021 30 November 2022 Option Year Three: 1 December 2022 30 November 2023 Option Year Four: 1 December 2023 30 November 2024 2. General Requirements: It is understood that all Contractors submitting a response to this Request for Quote will have an established business with five (5) years of experience in performing these types of services required and defined herein. The Contractor must also be trained and/or certified to perform services on all of the equipment listed in this PWS and elsewhere in the contract. All inspections, testing, and maintenance shall be performed in accordance with the Conformance Standards and the OEM recommendations. The Contractor shall provide the following fully qualified personnel, as defined in Section 3.0 of this PWS: one (1) Control Service Technician and one (1) Chiller/HVAC Mechanic to perform services at the Jesse Brown VA Medical Center. Contractor personnel shall be on site and available during normal hours of coverage (see Section 2.10). The HVAC Mechanic shall document work completed in a report submitted electronically to the COR. The contractor shall validate the accuracy of these reports and provide them to the COR on a monthly basis. Contractor shall provide: Controls Service Technician 40 hours per week. Contractor is to provide on-site Control Service Technician support a five days per week. Controls Service Technicians shall meet the follow minimum requirements: Under minimal supervision conduct preventive maintenance, repair, installation, and commissioning and general servicing of systems (including detailed troubleshooting of systems). Three years of industry experience or five years experience in servicing electronic and/or mechanical systems in the HVAC industry. PC experience required and the ability to program HVAC related software. Vocational school four-year program or an Associate degree in electronics, mechanical systems, computer technology, air conditioning or similar field. Chiller Mechanic 40 hours per week. Chiller Mechanic s site visits will coincide with the Control Service Technician s visits. Chiller Mechanic shall meet the following minimum requirements: Skilled craftsman in their trade with familiarity with split systems, CRUs, VAVs, chillers, AHUs, pumps, exchanges, fans, cooling towers and other applied equipment. Ability to perform end testing, servicing, wiring, and monitoring for HVAC equipment and related systems. Without supervision change belts and perform regular maintenance checks. At least five (5) years experience in commercial HVAC and mechanical troubleshooting. Mechanical Journeyman License. Appropriate licenses to work with refrigerants. A/C Shop training program: The contractor shall provide training for JBVAMC AC Shop maintenance and operation personnel. Training shall be quarterly, minimum of 3 sequential days per quarter. Class size will be ten (10) students. The contractor shall work with the AC shop supervisor to identify shop deficient areas and develop lesson plans. The contractor may use existing curriculum / courses from Johnson Controls Training Institute. The contractor shall provide to the COR for review and approval a draft training plan at the beginning of each contract year. Training will be conducted on-site at JBVAMC. Training will be a combination of class room and on-site demonstrations inside the mechanical spaces. The contractor shall maintain attendance logs for all sessions, and provide a copy of the logs to the COR and AC Shop Supervisor upon request. Provide certification of training to all qualifying participants. Additional Requirements: All replacement parts whose individual prices fall at or below $500.00 shall be included at no additional charge. All replacement parts whose individual prices exceed $500.00 shall be billed outside of this contract. Emergency services required outside of normal hours of coverage (see section 2.10) are not included in this contract. The Contractor shall provide overtime and premium rates for each position required under this contract. Although services required outside of normal hours of coverage shall not be required under this contract, rates provided may be used to negotiate one-time purchases for services outside of this contract on a case-by-case basis. The Contract includes the inspection, testing, servicing, maintenance, and repair labor on all existing building/room temperature control systems and devices including electric controls; electronic controls; pneumatic controls; drive motor controllers; heating, cooling and mixed air controls; temperature control panels and sub-panels; control air compressors and dryers; mixing box controls and associated damper controls; variable air volume controls and associated damper controls; including all controls for VAV boxes and associated damper controls, Dual Duct boxes and associated damper controls, and Fan Powered Boxes and associated damper controls; fan coil controls; coil controls; zone controls; and central fan system controls including associated supply, return and exhaust air duct system controls; and all similar control systems. Contractor tasking will be in accordance with the annual maintenance plan and emergent issues as identified by JBVAMC A/C Shop Supervisor and COR. Within seven (7) calendar days of the receipt of the award, the successful Contractor shall provide the Contracting Officer s Representative (COR) with a copy of the current calibration certification of all test equipment which is to be used by the Contractor on Jesse Brown VA Medical Center equipment. Notwithstanding, it is understood that the Contracting Officer (CO) or the COR shall have the right to obtain current certifications from the Contractor on a periodic basis upon request. Test equipment calibration shall be traceable to a national standard. Reports/Documentation: Daily Check in / check out is mandatory for services performed on site. This includes all repair labor, inspections, testing and/or maintenance services (See Reporting Requirements, Section 7.0). When the service is completed, the Contractor s representative or technician shall document services rendered on a formal Work Order, as approved by the COR. This detailed written report will be signed by the Contractor's lead service technician completely describing the specific tasks(s) performed, preventive maintenance performed, including the exact location of the system or device serviced, an itemized nomenclature list of all VA provided parts replaced (if any), including equipment identification (i.e., serial number), a description of all items requiring further attention, and the name(s) of the technician(s) performing the work. Regardless of any wording to the contrary which may appear on the Contractor's standard forms, those reports will be conditionally accepted by the Government subject to later inspection for compliance with established standards of quality. The report shall be signed or initialed by the COR or authorized representative. Electronic Service Log: The Contractor will devise and maintain an electronic service log for the tracking of all services performed under this Contract. This electronic report shall include service calls, updates, upgrades, inspections, testing, changes required to the system, and special service requests generated throughout the duration of the contract. This log can be web-based for easy access by the Government and the Contractor when needed. The Contractor shall provide a sample of the above log with their initial offer. This log may be revised/edited upon request of the Government in order to include any additional information desired over the course of the contract. Documentation of all services performed is a critical element to the success of this contract. Defective Parts: It is understood that at any time that the Contractor is requested to replace a defective part during any service or maintenance required, that the old part shall remain the property of the VA and disposal of same will be determined by the COR. All defective parts and system components removed shall be turned in to the COR. Contingency Plan: Contractor shall submit a contingency plan within fifteen (15) calendar days of contract award to ensure all services will be performed in a timely manner and make available upon request by the CO or COR at any time during this contract performance. Hours of Coverage: Normal hours of coverage are Monday through Friday from 7:30 AM to 4:00 PM, excluding Federal holidays. All service/repairs labor including preventive maintenance, inspections, and testing will be performed during normal hours of coverage unless requested by the COR and approved by the CO. For the Temperature Controls Systems and components, the Contractor shall be able to provide emergency services and repairs 24 hours a day, 7 days a week, and 365 days per year. The pricing/cost labor rates for these services shall be in accordance with the Price/Cost Schedule in Section B.4 of the contract. Response Time: Contractor's must respond with a return phone call response to the COR or his/her designee within one (1) hour for critical patient care areas and Veterinary Medical Unit and two (2) hours for non-patient care area after receipt of telephoned notification 24 (twenty-four) hours per day. If the problem cannot be corrected by phone, the FSE shall commence work (on-site physical response) within 4 (four) hours and will proceed progressively to completion without undue delay (unless authorized a delay by the CO or COR). For example, if hours of coverage are 7:30 AM to 4:00 PM, Monday thru Friday, a 4-hour response means, if a call is placed at 4:45 PM, Monday August 10th; the contractor must start on-site service by 8:45 PM, Monday, August 10th. The Contractor is required to affirm their ability to meet these response times in their initial offer. The COR and alternates have authority to approve/request services calls which result in no additional costs to the Government. The CO must approve and authorize in writing any services calls resulting in additional costs to the Government. These requests and authorizations shall be completed via contract modification. The VA observes the following days, listed in table below, and any other day specifically declared by the President of the United States, to be a Federal Holiday. When a holiday falls on a Sunday, the following Monday shall be observed as a legal holiday by Government agencies. When a holiday falls on a Saturday, the preceding Friday shall be observed as a legal holiday. If the contractor is scheduled for a service visit on a holiday the contractor, with the COR s approval, shall reschedule the visit to another day. Services/Qualifications: All work under this contract must be performed by fully trained and qualified Journeyman Level technicians/mechanics with five (5) years of experience, directly employed by the Contractor, experienced in the installation, repair, servicing, troubleshooting, and maintenance of the types of temperature control systems and devices specified. The assignment of trainees, helpers, or persons of similar skill levels to perform work under the provisions of this contract is strictly prohibited. Contractor is to provide trained and certified technicians that can service Johnson Controls Metasys system with familiarity with LON, BACNET and N2 protocols, shall meet qualifications and standards, equivalent to those prescribed by the industry and local practices as further defined in Conformance Standards. Work performed outside the normal hours of coverage at the request of the COR and CO in the event of an emergency, shall be priced in accordance with the Price/Cost Schedule in Section B.2 of the contract. Transportation and any other miscellaneous costs shall be included in the hourly rates provided with the Contractor s initial offer. These services are not within the scope of this contract and therefore shall be treated as separate requirements. The purpose of establishing off-hour rates is purely to provide streamlined emergency services in the event they are necessary. The Contractor shall not in the course of performing required service or under any other circumstances shut down all or any part of the supply, return or exhaust air fan systems without the expressed authorization of the Foreman, JBVAMC HVAC shop, or the COR during normal business hours or the on duty JBVAMC HVAC Mechanic after normal business hours and on weekends and holidays. 10. Specific Tasks: Required Preventive Maintenance (PM) servicing of the specified temperature control systems and control system devices must include accomplishment of all the tasks listed below for each type of part, component, assembly and sub-assembly to be serviced as well as all other such tasks recommended by the part, component, assembly or sub-assembly manufacturer and otherwise necessary to maintain the temperature control systems and devices in satisfactory operating condition. The Contractor shall perform regular and systematic preventive maintenance. Contractor shall service each existing building/room temperature control system and device in each of the Buildings at JBVAMC. This is to be performed IAW OEM specifications/recommendations. For example, upon inspection, when the Contractor discovers a control valve actuator not performing properly, the Contractor shall attempt to perform a repair with tools. If the part is found to be defective, the Contractor shall request approval from the COR to make the replacement or order the part. If the parts are made available at that moment, then the Contractor shall replace these components during the scheduled maintenance. If parts are unavailable, the repair will be performed as soon as the replacement parts are made available to the Contractor. In the event that a utility shutdown is required to perform a particular repair, the Contractor will need to coordinate a shutdown with the COR. This shutdown request must be submitted in writing 14 calendar days prior to the scheduled shutdown unless the urgency of the situation requires immediate service/repair. Preventive Maintenance (PM): The Contractor shall perform PM services to ensure that equipment listed in the PWS or Appendices performs in accordance with the Conformance Standards and/or the manufacturer's specifications. The Contractor shall utilize procedures and checklists with worksheet originals indicating work performed and actual values obtained (as applicable) and provide these documents to the COR at the completion of the PM. The Contractor shall provide a written and electronic plan of Preventive Maintenance and Inspections (PMI), which shall be submitted within 30 calendar days from date of award. This plan shall include an itemized list of the procedures to be performed, including electrical safety. PM services shall include, at a minimum, the following: Inspecting and cleaning of equipment. Reviewing operating system software diagnostics to ensure that the system is operating to the manufacturer's specifications. Calibrating and lubricating the equipment. Performing remedial maintenance of non-emergent nature. Testing and replacing faulty and worn parts and/or parts likely to become faulty, fail or become worn. All replacement parts, components, and materials are to be provided by the VA. Inspecting and repairing, where possible, existing electrical wiring and cables for wear and fraying. Inspecting, and replacing where indicated, all mechanical components which may require replacement. All replacement parts, components, and materials are to be provided by the VA. Returning the equipment to the operating condition as defined in the Conformance Standards OR to the manufacturer's specifications. Providing documentation of services performed. Preventative Maintenance services shall be performed in accordance with, and during the hours defined in, the preventive maintenance schedule established herein. All exceptions to the PM schedule shall be arranged and approved in advance with the COR. Contractor shall meet monthly to review the status of work being performed in accordance with the Preventative Maintenance Schedule and Plan with the COR. There shall be no additional charges for services, manuals, or tools required to successfully complete scheduled PM included within this contract, unless they are proprietary or specifically stated in writing otherwise. Controls: Electric/Pneumatic Controls (e/P), including relays, switches, pressure transmitters, in-duct smoke detectors, differential pressure transducers, hot water controls, thermostats, humidistat s, damper actuators, valve actuators, receiver controllers, pressure regulators, gauges, electrical circuit boards and similar type components. Clean as necessary; check control point settings and reset as necessary; check control air pressure and inspect for moisture; check nozzles and restrictors of all sensors, thermostats, humidistat s and pressure controllers; calibrate and check the throttling range of all sensors, thermostats, humidistat s and pressure controllers; inspect all control air piping and piping connections; inspect and calibrate all pressure gauges; inspect all wiring connections and contacts; check proportional band, reset and ratio settings. Electronic/DDC Controls: Clean as necessary; inspect all wiring, tighten loose connections, inspect linkages and connections; clean contacts; check control point settings and recalibrate as necessary; check complete sequence of operation. Single Zone Electronic Controls: Systems that have a single zone, or that have a thermostat controlling and air-cooled condensing unit shall have the thermostat included as part of the maintenance contract. Heating, Cooling and Mixed Air Controls: Clean as necessary; check proportional band, reset and ratio settings; check all sensors; check operators for response to pressure changes from controllers; check complete sequence of operation. Temperature Control Panels and Sub-Panels: Clean cabinets and components as necessary; inspect for leaks, clean/replace pneumatic air filters as necessary; check and calibrate all meters and gauges; check complete sequence of operation. Control Air Compressors: Drain tank and check traps; change oil and check oil pressure; replace all belts (labor only); replace sheaves (labor only) as necessary; replace intake and discharge air filters (labor only); inspect dryers; check pressure reducing valve setting; inspect and test high pressure safety relief valve; inspect and test pressure switches; check starter; calibrate pressure gauges; inspect control valve packing; time compressor operation. Variable Air Volume Box Controls: Inspect for proper operation of the integral volume controllers and adjust as necessary. All terminal air boxes: Inspect for proper operation of the integral volume controllers and adjust as necessary. Drive Motor Controllers: Clean components as necessary; check complete sequence of operation. Coil Controls: Inspect and tighten all electrical connections, relays, contactors and operating controls. Zone Controls: Clean as necessary; calibrate controllers; check switches and control valves; check pilot positioners; check auxiliary control devices. Replace zone thermostats if not operational (labor only). Central Fan System Controls, including associated supply, return and exhaust air duct system controls: Check complete sequence of operation; inspect and tighten all electrical connections and contactors; inspect, lubricate and check operation of dampers and damper linkage, including smoke and fire dampers; inspect, check operation and calibrate damper operator positive positioners; inspect operation of steam humidification system and adjust as necessary; check control valves; check all auxiliary control devices. For air systems controlled by Variable Frequency Drives (VFD), also inspect and service the VFD. As previously stated, none of the supply, return or exhaust air fan systems serving any of the specified buildings may be shut down completely or in part for any reason without the expressed authorization of the Foreman, JBVAMC A/C Shop, or his designee, during normal business hours or the on duty JBVAMC A/C Shop AC Mechanic after normal business hours and on weekends and holidays. Variable Frequency Drive controls (VFD s): For each drive manufacturer, perform recommended maintenance as outlined in the manufacturer s Operation and Maintenance manuals. In addition, check all sequences of control. 11. Contractor Responsibilities: a. Contractor will attend a pre-work orientation meeting prior to the commencement of work on site. The VA will schedule this meeting and it will include, at a minimum, discussion of the following topics: (VA will provide information to the Contractor regarding these topics and will document the meeting): Contract Terms and Conditions Contractor Post-Award Deliverables Contractor Administrative Requirements Performance Monitoring Security & Badging Procedures Fire and Safety Infection control Disaster procedures Patient Safety VA Handbook 7176 operational requirements Joint Commission Inspector General b. The Contractor will be responsible to ensure that Contractor employees coming to the work site will receive the information required above. The Contractor will be responsible to ensure Contractor employees providing work on this contract are fully trained and completely competent to perform the required work. The Contractor's employees shall wear visible identification and company uniforms approved by the COR, at all times while on the premises of the VHA. All contractors employees shall process through human resources for fingerprinting and issuance of PIV badging. Deliveries only at the Polk Street dock, and coordinated with the COR. At time of delivery: provide vehicle information to the VA Police and remove the vehicle as soon as delivery is complete. Move materials in public corridors during low patient time and performed between 6 pm through midnight concluding by 6 am on the following day. Contractor personnel may flex hours to move material through the hospital with written approval of the COR. No on-site parking or parking in the VA garage. Utilize street parking or other parking structures such as the Cook County Juvenile Parking Garage at their daily rate. The existing JB VAMC parking garage is at capacity and reserved for patients and staff. The Government will not invalidate or make reimbursement for parking violations of the Contractor under any conditions. Contractor shall use Freight Elevators. Do not move materials and employees in passenger elevators unless written approval provided by COR. Smoking is prohibited inside any buildings at the VHA. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court. All Contractors and Subcontractors performing services for the Government shall comply with all Occupational Safety and Health Administration (OSHA); State, County, and Municipal Safety; Occupational Health Standards; and any other applicable rules and regulations. Also, all Contractors and Subcontractors shall be held responsible for the safety of their employees and any unsafe acts or conditions that may cause injury or damage to any persons or property within and around the work site area under this contract. All ladders, scaffolding, tools, equipment, personal protective equipment, etc. shall be OSHA approved for the work to be performed. Contractor shall use caution signs as required by OSHA Regulations. Caution signs shall be on-site on commencement of Contract. No music radios. Two-way communications radios are allowed where broadcast frequency are approved by COR. Immediately stop work and notify the COR if asbestos is noticed or suspected in the area of work. All work in the JB VAMC crawl spaces shall be planned and carried out as OSHA Permit Required Confined Space. The Contractor shall purchase and issue all chemicals in their original containers. Materials that require precautionary warnings shall have affixed to all containers such labels or markings as are prescribed by law, regulatory agencies, or this contract. Any violation of OSHA may be subject to default action. Contractor shall furnish to the COR two (2) copies of Safety Data Sheets (SDS), for all products proposed for use, a minimum of seven (7) days prior to beginning service, for approval. Contractor must update copies of the SDS on an annual basis. In addition, each time a new chemical product is proposed to be introduced into the facility, a copy of that product's SDS must be provided to the COR for approval, prior to the product being used at the facility. 12. Reporting Requirements/On-Site Work at Jesse Brown VA Campus: a. Prior to beginning any required preventive maintenance services or performing repairs between the hours of 7:30 AM and 4:00 PM Monday through Friday, excluding Federal holidays, the Contractor's service technician(s) must report to the A/C Foreman, JBVAMC A/C Shop, Building 2, Room 011A, Lower-Level, or his/her designee, and advise him/her as to the nature and location of the work to be accomplished. Upon completion of work during regular business hours, the Contractor's service technician(s) must again report to the A/C Foreman, Air Conditioning and Refrigeration Unit, or his/her designee, and advise him/her of both work completed and any problems identified which were not corrected. b. Prior to performing work after regular business hours and on weekends and holidays the Contractor's service technician(s) must sign in at the AOD and then coordinate his/her activities with the on duty JBVAMC A/C Shop Air Conditioning Equipment Operator. Upon completion of the work the Contractor's service technician(s) must sign out at the AOD and then promptly and directly depart the Hospital grounds. c. The VA COR for this contract shall be from the Engineering Department at Jesse Brown VA Medical Center and shall be appointed at time of award. Due to the size and complexity of this acquisition, there will be an alternate COR assigned to this contract. 13. Conformance Standards: All work shall be performed and equipment shall function in conformance with all VA Policy and Procedures, safety standards, manufacturer's/industry standards, the latest published edition of UL, EPA, NFPA-99, FDA, OSHA, TJC, applicable local and national standards, and VA Policies/regulations for facilities and patient care. Contractor shall provide UL classification used (if applicable) for all products/materials supplied by the Contractor. 14. REFERENCES/STANDARDS: Contractor and/or subcontractors performing work under this contract shall provide services in accordance with the following, latest standards from the list identified below: AABC - National Standards for Total System Balance. ADC Test Code for Grilles, Registers, and Diffusers. AMCA Field Performance Measurement of Fan Systems. ASHRAE - HVAC Applications Handbook; Chapter 37, Testing, Adjusting and Balancing. ASHRAE/ANSI - Practices for Measurement, Testing, Adjusting and Balancing of Building HVAC&R Systems. Safety Data Sheets (SDS): Prior to commencement of services the Contractor shall provide the Safety Data Sheets in accordance with VA and OSHA regulations. 16. Definitions/Acronyms: CO - Contracting Officer COR - Contracting Officer's Representative PMI - Preventive Maintenance Inspection. Services which are periodic in nature and are required to maintain the equipment in such condition that it may be operated in accordance with its intended design and functional capacity with minimal incidence of malfunction or inoperative conditions. FSE - Field Service Engineer. A person who is authorized by the Contractor to perform maintenance (corrective and/or preventive) services on the VHA premises. ESR - Vendor Engineering Service Report. A documentation of the services rendered for each incidence of work performance under the terms and conditions of the contract. Acceptance Signature - VA employee who is authorized to sign-off on the ESR which indicates that the PM has been concluded or is still pending completion, or that the Emergency Repair has been accomplished or is still in a pending status. Authorization Signature - COR's signature; indicates COR accepts work status as stated in ESR. NFPA - National Fire Protection Association. Condition of Equipment: Upon award, the Contractor accepts responsibility as is for all temperature control equipment specified in the contract and Appendices.
