The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. The Great Lakes Acquisition Center (GLAC) is soliciting businesses to provide tools, materials, components, labor and supervision for the tri-annual maintenance, service and testing of the fifty-two (52) mid-voltage vacuum circuit breakers, which serves all electrical power distribution systems at Jesse Brown VA Medical Center, Chicago, Il. It is understood that all Contractors submitting a response to this Sources Sought will have an established business with three (3) years of consecutive experience in performing these types of services at healthcare facilities. The Contractor must also be licensed and authorized to perform services on all the equipment listed. The following NAICS Code and Small Business Size Standard has been identified for this requirement: 238210 Electrical Contractors and Other Wiring Installation Contractors, with a Size Standard of $15.0 Million in annual revenue. Interested parties shall provide a brief overview of their firm s capabilities and past experience with projects similar in size and scope of this project. Capability statements shall also include: DUNS number Organization Name and Assumed Name/dba (if applicable) Organization Address Point of Contact (including name, title, addresses, telephone number, fax number and Email address) Business size (small business or other than small business) Socio-economic status (If SDVOSB or VOSB provide SDVOSB or VOSB CVE verification) Organization experience in performing these types of services Copies of applicable certifications and/or accreditations (All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments.) STATEMENT OF WORK Place of Performance: Buildings 42, 21, 1C, and 40 Jesse Brown VA Medical Center 820 S. Damen Ave Chicago IL 60612 Part 1: Project Scope of Work Overview: Contractor shall provide all tools, materials, components, labor and supervision for the Tri-Annual Maintenance, service and testing of the FIFYTY TWO (52) MID VOLTAGE VACUUM CIRCUIT BREAKERS Serving all Electrical Power Distribution Systems at Jesse Brown VA Medical Center Chicago, Il. Testing and documentation shall comply with the requirements of the Institute of Electrical and Electronics Engineers (IEEE) Standard 1584; NFPA 99 4.4.4.1.2.2, NFPA 6.7.4.1.2.1, NETA 7.5.3, NETA 7.6.1.1 for analyses, labeling and reporting current facility conditions, including additions, upgrades and other changes that occur following completion of all circuit breaker analyses. Part 2: Period of Performance (POP) and Project Construction Cost Magnitude: Complete all work including submittals, reviews and quality control within 183 calendar days from Notice to Proceed (NTP). Part 3: Detailed Project Scope of Work: Work is to be scheduled to meet existing NETA testing standards and JBVAMC testing compliance time line. JBVAMC Facility Management shall approve a schedule 21-days in advance to any scheduled power shutdowns. Circuit Breaker testing shall have a written Standard Operation Procedure, to be approved by the Chief Engineer. Power interruptions shall not exceed 15-minutes on any Mid-Voltage power distribution branch distribution. Fifty-Two (52) Mid Voltage Circuit Breakers to be tested; count and location: Bldg. 21 (22)-5kv Eaton/Cutler-Hammer Type 50VCP-W Circuit Breakers. Bldg. 1C 2nd. Flr. (43south) (3)-Square D Med. Volt Type VR Bldg. 1C 1st. Flr. South (9)-5kv Eaton/Cutler-Hammer Type 50VCP-W Bldg. 42 (9)-5kv Square D Med. Volt. Type VR Bldg. 40 Bed Pavilion (9)-5kv Eaton/Cutler-Hammer Type 50VCP-W Hours of Operation. Testing circuit breaker shall be scheduled 14-days in advance, Monday through Friday from 1800-0500 hrs. CST, Saturday through Sunday 2400-hrs.CST including holidays. All testing that requires electrical shutdowns will be performed after normal business hours of coverage unless requested or approved y COR or designee. Holidays include: New Years' Day Martin Luther King Day Presidents' Day Memorial Day Independence Day Labor Day Columbus Day Veterans' Day Thanksgiving Day Christmas Day And any other day specifically designated as a national holiday by the President of the United States. Place of Performance. Jesse Brown VA Medical Center 820 S. Damen Ave Chicago, IL 60612-3728 Period of Performance. Base Year: NTP- 182-DAYS Performance Monitoring. Various methods exist to monitor performance. The COR shall use the surveillance methods listed below in the performance monitoring of this contract. Direct Observation-(COR will have 100% surveillance for all utility shutdowns and facility electrical power functionality.) Periodic Inspection- COR WILL BE ON STATION TO INSURE THAT SOP IS APPLIED AND NO UTILITY POWER OUTAGE EXCEEDS 15-MINUTES.) Completed testing documentation shall be submitted to the COR weekly, every seven days (Julien Calendar). ACCEPTABLE PERFORMANCE The Government shall document positive performance. Any report may become a part of the supporting documentation for any contractual action. Utility electrical power shutdowns at the Jesse Brown VA Medical Center shall be scheduled 21-days in advance and approved by the Chief Engineer. Contractors shall sign-in and obtain a badge at the information desk Ogden Entrance and return the badge at the end of the day. Prior to starting circuit breaker testing, the contractor s technicians shall report in person to the COR, submit a written SOP outlining the schedule for utility shutdowns to include approval by Chief Engineer for services performed and completed to include exact switchgear, locations, and services to be impacted with scheduled power outages. For services performed outside normal working hours, the contractor s Technicians shall call the COR representative at Jesse Brown VA Medical Center. After all work is completed testing documentation shall again be reported to the COR where the Technician shall submit the daily progress reports required above. UNACCEPTABLE PERFORMANCE When unacceptable performance occurs, the COR shall inform the contractor. This will normally be in writing unless circumstances necessitate verbal communication. In any case the COR shall document the discussion and place it in the COR file. When the COR determines formal written communication is required, the COR shall prepare a Contract Discrepancy Report (CDR) and present it to the contractor's program manager. The contractor shall acknowledge receipt of the CDR in writing. The CDR will specify if the contractor is required to prepare a corrective action plan to document how the contractor shall correct the unacceptable performance and avoid a recurrence. The CDR will also state how long after receipt the contractor has to present this corrective action plan to the COR. The Government shall review the contractor's corrective action plan to determine acceptability. Any CDRs may become a part of the supporting documentation for any contractual action deemed necessary by the CO. Security Requirements. Vendor will not have access to VA Sensitive Information, patient records, or data. For repairs or services performed during normal work hours, 7:00 a.m. to 4:40 p.m. at the Jesse Brown VA Medical Center, contractors shall sign-in and obtain a badge and return the badge at the end of the day. Prior to starting and circuit breaker testing, the contractor s technicians shall report in person to the COR, submit a written SOP outlining the services performed and completed to include locations and services to be impacted with scheduled power outages. For services performed outside normal working hours, the contractor s Technicians shall call the COR representative at Jesse Brown VA Medical Center. After all work is completed testing documentation shall again be reported to the COR where the Technician shall submit the daily reports required above. Government-Furnished Equipment (GFE) / Government-Furnished Information. None. Other Pertinent Information or Special Considerations. All work shall be performed by personnel qualified to work on the specific equipment. All work performed shall comply with IEEE, NFPA, OHSA, and TJC guidelines and shall be designed to assure that the Jesse Brown VA Medical Center is provided power with the least power interruptions REQUIRED REGISTRATION WITH CONTRACTOR PERFORMANCE ASSESSMENT SYSTEM (CPARS). As prescribed in Federal Acquisition Regulation (FAR) Part 42.15, the Department of Veterans Affairs (VA) evaluates contractor past performance on all contracts that exceed the thresholds outlined in FAR Part 42.15, and shares those evaluations with other Federal Government contract specialists and procurement officials through the Past Performance Information Retrieval System (PPIRS). The FAR requires that the contractor be provided an opportunity to comment on past performance evaluations prior to the posting of each report. To fulfill this requirement VA uses an online database, the Contractor Performance Assessment Reporting System (CPARS). The CPARS database information is uploaded to the Past Performance Information Retrieval System (PPIRS) database, which is available to all Federal agencies. Each contractor whose contract award is estimated to exceed the thresholds outlined in FAR Part 42.15 is required to provide to the contracting officer contact information for the contractor s representative with their response to the solicitation. The contractor is responsible to notify the contracting officer of any change to the contractor s representative during the contract performance period. Contractor s representative contact information consists of a name and email address. The Government will register the contract within thirty days after contract award. For contracts with a period of one year or less, the contracting officer will perform a single evaluation when the contract is complete. For contracts exceeding one year, the contracting officer will evaluate the contractor s performance annually. Intermediate reports will be filed each year until the last year of the contract, when the final report will be completed. Each report shall be forwarded in CPARS to the contractor s designated representative for comment. The contractor s representative will have thirty days to submit any comments and return the report to the VA contracting officer. Failure by the contractor to respond within those thirty days will result in the Government s evaluation being placed on file in PPIRS without contractor s comments. Part 4: Submittals and Milestone Deliverables: The Contractor and Jesse Brown VA Electric Shop Field Supervisor shall attend a Pre-Maintenance Safety Meeting, to discuss the service details, Generator Running and testing to be scheduled Monday-Friday after 6:00pm. Overtime should be included in finalize quote. ACCEPTANCE AND CLOSE OUT Acceptance Criteria for the work: Demonstrate the operation of the system Provide factory startup and certification of all systems. Schedule the demonstration with the COR 21 days in advance. Part 5: Applicable Performance Standards All work shall comply with all VA Policies including and not limited to: VA Construction Safety and VA Infection Control Policies. The Prime Contractor shall have a Competent Supervisory Person on site at when any worker(s) or sub-contractors are present. All persons working on any electrical systems rated 50 volts or more shall have the minimum qualification as a State Licensed Master Electrician or Registered Journeyman Electrician, all activities shall follow the safe work practices in compliance with NFPA 70E Electrical Safety in the Workplace. The Contractor shall provide mitigating activities as outlined in the CONSTRUCTION SAFETY PRECAUTIONS including but not less than the following: Fire extinguishers, pedestal mounted in work No trash, used packaging or construction spoils shall be stored on site. Remove each day using covered gondolas. Broom sweep the work area at the end of each day. All work will be provided with 1-year parts and labor guarantee from date of acceptance by the VA. Part 6: Jesse Brown VAMC Additional Requirements Comply with all Jesse Brown VAMC site specific requirements and SOPs including and not limited to: Key Policy, Badging Policy, Rules of Behavior, and Smoking Policy. Normal circuit breaker testing hours are 9:00 pm to 5:30 am Monday through Friday. Normal circuit breaker testing weekend hours 6:oopm to 6:00am Saturday through Sunday. Work hours can be modified only by written approval of the COR. The Contractor shall check in (in person) with the Administrative Officer of the Day (AOD) which is in Patient Admitting for any work between 6:00 pm through midnight to 6:00 am or any time on weekends. Provide the name and cell number of the on-site supervisor to the AOD. At the completion of the work, check out with the AOD. All building systems will be maintained in full operation always: Request system shutdowns in writing not less than 21 calendar days in advance. Shutdown work shall be during low patient treatment time and is subject to the be rescheduled so not to interfere with veteran patient treatment.
