The contractor must provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and vehicles needed to deliver non-emergency medical transport (NEMT) services for Cleveland VA and surrounding CBOCs. Services must comply with the Ohio Administrative Code, EMFTS requirements, and applicable federal, state, and local laws, rules, regulations, and procedures. The contractor is responsible for veteran pick-up and delivery from community hospitals, VA facilities, patient residences, and other authorized locations within and outside the service area, supporting Cleveland VA Medical Center (Wade Park) and CBOCs in Akron, Canton, Ravenna, New Philadelphia, Mansfield, Lake County/Willoughby, Sandusky, Lorain/Sheffield Village, Parma, East Liverpool, Warren, and Youngstown, Ohio. The performance periods are a base period from 1 February 2027 to 31 January 2028, Ordering Period #1 from 1 February 2028 to 31 July 2029, and Ordering Period #2 from 1 February 2029 to 31 June 2030 as stated.
