Employee Assistance Program (EAP) Services for Dayton VAMC and CBOCs The purpose of this sources sought notice is to seek responses from all qualified prospective offerors interested in the forthcoming solicitation for the subject requirement summarized as follows: Short Title: Employee Assistance Program (EAP) Services for Dayton VAMC and CBOCs NAICS 624190: Other Individual and Family Services Place of Performance: Contractor facility and Dayton VAMC as necessary for site visit(s) Anticipated Periods of Performance: The performance periods include a base year and four (4) options years if exercised summarized as follows: Base Year (CLIN-0001): 1 November 2024 to 31 October 2025 Option Year One (CLIN-1001): 1 November 2025 to 31 October 2026 Option Year Two (CLIN-2001): 1 November 2026 to 31 October 2027 Option Year Three (CLIN-3001): 1 November 2027 to 31 October 2028 Option Year Four (CLIN-4001): 1 November 2028 to 31 October 2029 Attachment 1 for Reference: Draft version of the Performance Work Statement (PWS) is attached for your reference as appropriate. Attachment 2 for Completion/Signature: Statement of FAR 52.219-14 (Limitations on Subcontracting) Compliance for completion/signature. Attachment 3 for Reference: DOL WD 2015-4731-R24 Montgomery County OH 22Jul24 Request all prospective offerors interested in this requirement email their 1) company name, 2) UEI number, 3) CAGE code, 4) business socio-economic size status, 5) signed and fully completed statement of FAR 52.219-14 Limitations on Subcontracting Compliance and 6) capabilities statement describing how specifically your company intends to meet all requirements specified in the PWS in Adobe PDF format to bradley.burhite@va.gov not later than 2 October 2024 at 1400-EST. In addition, please note that a signed and full completed copy of the FAR 52.219-14 (Limitations on Subcontracting) compliance letter is required to be considered responsive to this source sought notice.
