Sources Sought Notice Page 5 of 5 Page 1 of 5 SOURCES SOUGHT NOTICE For General Surgeon Physician Services to include on-call for VA Northern Indiana Health Care System in Fort Wayne, Indiana This is a Sources Sought Notice. This advertisement is not a solicitation for offers, nor is it a request for quotes. A request for quotes may be issued at a later date. This Sources Sought/Market Survey Notice is being conducted by the Network 10 Contracting Office (NCO 10), PO Box 492, Ann Arbor, MI 48106 to identify sources that can provide on-site General Surgeon Physician Services to include on-call to eligible beneficiaries of the Department of Veterans Affairs Medical Center, Northern Indiana Health Care System, in Fort Wayne, Indiana. The physician(s) will evaluate and treat patients in the General Surgery department to include outpatient consultation, e-consults, virtual care, telephone, and CVT. The physician(s) will perform Operating Room procedures. Emergency responses may be required intermittently within the facility in which the General Surgeon is responsible to respond timely. Hours of coverage shall be from 7:30am through 4:00pm Monday through Friday; no weekends and no Federal Holidays. Hours of coverage may vary dependent upon the needs of the organization. The contractor hours may be reduced should the facility appoint their own medical staff. The contractor is responsible for provided appropriate medical staff coverage for all vacant shifts not filled by VA medical staff. All Contractor s physician(s) shall be Board Certified by the ABS http://www.absurgery.org/, and be currently certified in BLS, ACLS or equivalency. and contractor s physician(s) shall maintain certification throughout the administration of the contract. Period of Performance: 12 months from date of Award plus four, 12-month option periods.
