This notice is to identify entities that have the resources and capabilities to provide onsite Perfusionist Services consisting of one Chief Perfusionist, two Staff Perfusionists, and one PRN Perfusionist (3.5 FTE) to eligible beneficiaries of the VA Northeast Ohio Healthcare System (VANEOHS), located at 10701 East Blvd, Cleveland, OH 44106. All Perfusionists shall be a graduate of an accredited Allied Health Education Program School of Perfusion Technology and shall be certified by the American Board of Cardiovascular Perfusion. Experience shall be in the following areas to include but not limited to: open heart surgery, heart lung machine, membrane oxygenation, left/right heart bypass, intra-aortic balloon pumping and centrifugal ventricular assist device. All interested contractors are invited to provide information to contribute to this market survey. The Principle NAICS Code is 621111, Office of Physicians (except Mental Health Specialist) with a business size standard of $16.0 Million. The anticipated performance period will be February 15, 2025 February 14, 2026, with the possibility of four (4), one (1) year option renewals to be exercised at the Governments discretion. Responses will not be considered as proposals, and no award will be made to interested parties in response to this announcement. Contractors having the skills and capabilities necessary to perform the stated requirements should submit a response of no more than 10 pages in length. The Government will not review any other data or attachments that are in excess of the 10 pages. Companies should provide clear and unambiguous evidence to substantiate their capability to fulfill these requirements. Interested and Capable Companies shall furnish the following minimum information: Business name, SAM Unique Entity Identifier (UEI) Number, person of contact name including telephone number, address, email address, and company website. Business size and small business size classification if applicable (e.g. Service Disabled Veteran-Owned, Veteran-Owned, Women-Owned, Disadvantaged Small Business, 8(a) etc.). Is your company Veteran Small Business Certification (VETCert) verified? Federal Supply Schedule (FSS) Contract, list the GSA Contract Number and relevant SINS. Capability statement providing clear evidence to substantiate the capacity to fulfill this requirement as stated above. A summary of previous experience providing Perfusionist Services for the VA or other healthcare entities.
