REQUIREMENTS: Department of Veterans Affairs Network Contracting office (NCO) 10, 3140 Governor s Place Blvd. Suite 210, Kettering OH 45409-1337, intends to solicit offers from a certified Arjo installer for Installation of their Arjo TackLift ceiling mounted lift/scale system to accommodate patients with spinal cord injuries accommodating patients up to 600 lbs. FOB: Destination Place of Performance: Department of Veteran Affairs Aleda E. Lutz VA Medical Center 1500 Weiss Street Saginaw, MI 48602 USA Contractor shall furnish all supervision, labor, travel, equipment, parts, and supplies excluding the listed items purchased already by Aleda E Lutz VA Medical Center: (MOTOR: MS205-01-01 quantity 1, SPREADER BAR: 700-19465 quantity1, SCALE: 700-19485 quantity 1, 3 METER 90MM KWIKTRAC: 200.12810 quantity 2, 4.5 METER 140MM KWIKTRAK quantity 1, HARDWARE KIT quantity 1) to complete the installation. Listed equipment is located at the Aleda E Lutz VA Medical Center, Building 1, room 1-4263, 1500 Weiss Street, Saginaw, Michigan 40602. Service will be provided between 8:00am to 6:00pm, Monday through Friday, excluding Federal holidays. 4. Specific Tasks. Contractor will assemble and mount the lift and track system in room 1-4263 to accommodate lifting patients up to 600lbs. FMS will install an electrical outlet for said lift system in coordination with the Contractors. FMS will be responsible for drop ceiling/tile work following installation. Only Manufacturer Trained Service Technicians or Manufacturer Authorized Service Technicians will perform installation. The Contractor shall obtain all necessary licenses and/or permits required to perform this work. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to any contractor representative, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by the contractor s fault or negligence. The Contractor will not discuss any patient issues that were observed during the work. The Contractor meets the definition of Business Associate. Therefore, a Business Associate Agreement is required to comply with both the Health Insurance Portability and Accountability Act (HIPAA) Privacy and Security regulations. Contractor must be registered with https://www.sam.gov Any contractor that believes they are capable and desires to claim preference for small business status must be registered with the SBA and be able to be located through SBA - Dynamic Small Business Search. Any contractor that believes they are capable and desires to claim preference for veteran owned small business status must be registered with in VetCert at Veteran Small Business Certification (sba.gov) as an SDVOSB or VOSB.
