This notice is intended strictly for market research. The purpose of this Sources Sought notice is to determine the following: The interest and capability of small business, especially Service-Disabled Veteran Owned Small Business and Veteran Owned Small Business. Whether the business source is a manufacturer or authorized distributor, verified by an authorization letter from the original equipment manufacturer; and Size classification relative to the North American Industry Classification System (NAICS) code 339112 for the proposed acquisition. The Department of Veterans Affairs, Network Contracting Office 10 is seeking potential sources for (16) Haag-Streit BQ900 LED Slit Lamp, Model #: HS0900.0011LED and (16) Haag-Streit Goldmann Applanation Tonometer, Model #: HS09004.4, brand name or equal, to be provided for the Louis Stokes Cleveland VA Medical Center, 10701 East Boulevard, Cleveland, OH 44106-1702. All items shall be new; refurbished/used items are not acceptable. Brand name or equal requirements: Purchase of 16 Haag-Streit BQ900 LED Slit Lamp, Model #: HS0900.0011LED and 16 Haag-Streit Goldmann Applanation Tonometer, Model #: HS09004.4 or equivalent. The contractor is responsible for the delivery of requested LED Slit Lamp and Tonometer and any compatible accessories. We are requesting brand name or equal and have provided the characteristics of the requested LED Slit Lamp and Tonometer below under specific tasks. We request that the contractor be capable of providing LED Slit Lamp and Tonometer s that can either meet or exceed the salient characteristics of the requested type. We are requesting consideration to replace these items with the same model recently purchased and deployed in other areas. The contractor shall provide the delivery of the requested. The requested slit lamp equipment solution shall provide, but not be limited to, the following functionalities/Capabilities: LED illumination head, Inclined Eyepiece with 20-degree upwards inclination to alleviate neck discomfort, Background illumination, Eyepiece with double cross hair reticule, Knob for beam splitter/camera on-off, Aperture control knob, Diffusor, Imaging Module 910, Shutter-release, Illumination control, Beam splitter for slit lamp system that permits selective diversion of 50% of light to secondary slit lamp system attachment (observation tube), Observation tube (length ~ 150mm) with adjustable eyepiece for attachment to beam splitter of slit lamp system to permit observation by trainees, Stereo Variator ability to reduce angle from standard 13 degrees to 4.5 degrees, Unit model slit lamp that can be mounted on existing ophthalmic instrument stands (Reliance 7800-IC Floor Unit ophthalmic instrument stand), Slit image width of 0 to 14mm continuous, Slit image length of 1 to 14mm continuous, Illumination field apertures of 8, 5, 3, 2, 1 & 0.2mm, Slit image radial range +/- 90 degrees, Swiveling of the slit illumination relative to the microscope axis: +/- 90 degrees horizontally and 0 to 20 degrees vertically, Filters: Including blue, red-free (green) and grey (10%), Light source: LED with 400-750nm range of spectral illumination, Microscope stereo angle of 13 degrees, Microscope magnification changer including 6.3x, 10x, 16x, 25x & 40x, Eyepiece Magnification: 12.5x, Range of adjusting eyepieces: +7 diopters to -7 diopters, Interpupillary distance range: 52 to 78mm, The Haag-Streit 900 BQ Slit Lamp is standardized equipment in all currently VAPORHCS eye exam rooms. The requested tonometer equipment solution shall provide, but not be limited to, the following functionalities/Capabilities: Goldmann style applanation tonometer for use with Slit Lamp System above, Measurement range 0 80mm Hg, and Measuring force Generated by leverage weight. All contractors that are interested in meeting this requirement are asked to submit the following information: Company Name, Company Address, DUNS Number, Company POC Name, Company POC Phone, Company POC Email, Business Size/Type under NAICS 339112, Country of Origin for proposed item GSA Contract Number (if applicable). If proposing equal items, provide detailed documentation showing how the proposed items are equal to the Avia brand. Additional information: All contractors must be registered with the System for Award Management (SAM) at https://www.sam.gov. Any contractor that believes they are capable and desires to claim preference for small business status must be registered with the SBA and meet the requirements of FAR 19.102. Any contractor that believes they are capable and desires to claim preference for Veteran Owned Small Business status must be registered with SBA Veteran Small Business Certification (VetCert) at https://veterans.certify.sba.gov/ as an SDVOSB or VOSB.
