Medical Physics Certification Services for Radiology and Quarterly Audits for Nuclear Medicine (DAY/COL/CIN/CLL) The purpose of this sources sought notice is to seek responses from all qualified, prospective offerors interested in the forthcoming solicitation for the subject requirement summarized as follows: Short Title: Medical Physics Certification Services for Radiology and Quarterly Audits for Nuclear Medicine (CIN/DAY/COL/CLL) NAICS 541690: Other Scientific and Technical Consulting Services Places of Performance: Dayton VAMC (DAY), Wylie P. Chalmers VAACC in Columbus (COL), Cincinnati VAMC (CIN) and Chillicothe VAMC (CLL) Anticipated Period of Performance: The performance periods for the base year plus four (4) options years are summarized as follows: Base Year: 25 February 2024 to 24 February 2025 Option Year One (OY1): 25 February 2025 to 24 February 2026 Option Year Two (OY2): 25 February 2026 to 24 February 2027 Option Year Three (OY3): 25 February 2027 to 24 February 2028 Option Year Four (OY4): 25 February 2028 to 24 February 2029 Attachment 1 for Reference: Performance Work Statement (PWS) Attachment 2 for Completion/Signature: Statement of FAR 52.219-14 (Limitations on Subcontracting) Compliance for completion/signature. Request all prospective offerors interested in this requirement email their 1) company name, 2) UEI number, 3) CAGE code, 4) business socio-economic size status, 5) signed statement of FAR 52.219-14 Limitations on Subcontracting Compliance and 6) capabilities statement to perform this requirement IAW the PWS in Adobe PDF format to bradley.burhite@va.gov not later than 29 November 2023 at 14:00-EST. In addition, please note that a signed copy of the FAR 52.219-14 (Limitations on Subcontracting) compliance letter is required to be considered responsive to this source sought notice.
