Employee Assistance Program (EAP) Services for Chillicothe VAMC and CBOCs The purpose of this sources sought notice is to seek responses from all qualified prospective offerors interested in the forthcoming solicitation for the subject requirement summarized as follows: Short Title: Employee Assistance Program (EAP) Services for Chillicothe VAMC and CBOCs NAICS 624190: Other Individual and Family Services Place of Performance: Contractor facility and Chillicothe VAMC as necessary for site visit(s) Anticipated Periods of Performance: The performance periods include a base year and four (4) options years if exercised summarized as follows: BASE YEAR:Â Â 09/15/2022 to 05/31/2023 OPTION YEAR ONE (1):Â Â 09/15/2023 to 09/14/2024 OPTION YEAR TWO (2):Â Â 09/15/2024 to 09/14/2025 OPTION YEAR THREE (3):Â Â 09/15/2025 to 09/14/2026 OPTION YEAR FOUR (4):Â Â 09/15/2026 to 09/14/2027 Attachment 1 for Reference: Draft version of the Performance Work Statement (PWS) is attached for your reference as appropriate.
