Volume unknown. The Contractor shall arrange with the COR a routine (non-emergency) service for the facility based upon volume. A pick-up schedule shall be established with the SJCVAC for a weekly service or on an on-call basis, as needed. If an unscheduled pick up is called in, contractor shall respond and complete pick-up within 2 hours after initial call is made. Contractor shall be notified when an extra pick-up or changes in pick-up days are required. Coordination for pick-up shall be made with the COR. Contractor shall submit to the COR a weekly schedule of services. How many and what type of containers are required to be provided by the contractor? The contractor shall furnish appropriate containers, pick-up, and disposal of infectious waste in accordance with OSHA regulations 29 CFR 1910.1030 and DOT regulation 49 CFR 173.134. Based on the estimated weekly infectious waste of Regulated Medical Waste (RMW) includes, but is not limited to items contaminated with blood and body fluids/waste, chemotherapy wastes, sharps, pathological and microbiological waste or other potentially infectious materials, pick-ups will be scheduled one (1) time per week at a minimum. Contractor shall be in full operations with four (4) each approved standard storage/shipping containers on site and ready for pick-up. Is there a specific method of destruction preferred/required? All hazardous and non-hazardous materials generated by SJCVAC shall be disposed of by the Contractor in accordance with current Federal, State and local guidelines governing regulated hazardous and non-hazardous chemical wastes. Contractor shall dispose of hazardous waste in a manner that leaves no future expense potential to the VA or the federal government. Do you want a quote in a price per pound? price per box?      Pricing should be monthly as laid out in the price schedule on the solicitation. What are the reporting requirements? Documentation from the Contractor: The contractor shall prepare and submit to the COR an original signed Certificate of Destruction for each pick up for each site within thirty (30) working days of each pickup. The contractor shall maintain proper records concerning each Certificate of Destruction issued. The Certificate of Destruction shall at a minimum contain the following: ·         Pickup/Delivery Location ·         Pickup/Delivery Date ·         Amount of waste (number of containers picked up and/or pounds) ·         Destruction Certification Document/Manifest Number ·         Destruction Certification Date ·         Any other information as required by the COR.
