This announcement constitutes the only solicitation; quotations are being requested and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2022-01, effective November 4, 2021. This solicitation is set aside 100% for SDVOSB. This requirement will be awarded on all-or-none basis. Background The Dayton VA Medical Center (VAMC), Community Living Center (CLC), requires the use of Long-Term Care Electric Beds to meet the needs of our patients. These beds will provide a safer environment for Veterans due to the low-rise, side-rail configuration and entrapment requirements. The CLC currently has 165 of these beds currently in use, and continuity and standardization of equipment type is required for this procurement. 2. Scope The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. Deliver quantity twelve (12) Umano Ook Snow medical beds, part number FL36OOK-SNOW to the Dayton VA Medical Center. Assembly and setup of 12 Ook Snow medical beds Configuration of the beds will be in accordance with the features listed on Umano Medical quotation number 17159. Deliver quantity twelve (12) Umano Custom Care Convertible Low Air Loss (LAL) Surface Mattresses, part number U-CL843629, dimensions 84 x36 x7 . Deliver quantity twelve (12) Control Units, part number U27686, for Umano Custom Care Convertible Low Air Loss (LAL) Surface Mattresses, part number U-CL843629. Provide twelve (12) 3 foot long power cords and twelve (12) sets of short hooks needed to mount the units. Removal of all packing materials Provide 3-year warranty on all parts and labor. Provide operator training as required. Provide two copies of service manuals for beds and control units. 3. Salient Characteristics EQUIPMENT SPECIFICATIONS The Contractor shall furnish all equipment and training when delivering the Long-Term Care Electric Beds and Mattresses. Product shall be clean and in good/working condition upon delivery. Contractor s employees shall be discreet, tactful, demonstrate concern, compassion, and patience with a high degree of professionalism and understanding. The location of delivery will be at the Dayton VAMC, CLC, 4100 West Third Street, Dayton, OH 45428. Training/In-Service. Contractor shall provide education to staff on proper bed controls, operation and safety measure upon delivery. Operator training shall be conducted to staff on proper use of equipment for 1.5 days as scheduled by CLC staff. Contractor shall have a training specialist. Equipment Specifications. The Government will issue a Firm-Fixed Price Purchase Order resulting from this solicitation to the responsible offeror whose offer conforms to the requirements within this solicitation and will be the most advantageous to the Government. Best Value will be how the quotes are evaluated. The following factors will be used in evaluation: A price comparison along with Technical Acceptability, Delivery, and Warranty. An Offeror s initial quote shall contain the best terms from a price and technical standpoint. Offerors are reminded that representatives from your company SHALL NOT contact any Dayton VAMC personnel to discuss this RFQ during the solicitation and evaluation process. All warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. The Contractor shall provide new items ONLY; no remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. Quote must be good for 90 calendar days after close of this Combined Synopsis Solicitation.
