The attached Spreadsheet has the 15 required columns highlighted. Please complete columns A, C, G, H, I, J, K, L, N, O, AI, AJ, BY, CC and CR. If a column request a number and if that column is not applicable, please enter 0 . If a column request a name and if that column is not applicable, please enter none You only need to complete the Items Tab. The other Tabs are only included for your assistance if needed.
