How is this additional fee calculated? On page 6, Section B.2, The emergency fee is removed from the solicitation. The pricing will be determined by a base rate charge plus a standard mileage charge. The base rate will be added on each end of the trip. Thus, a base rate will be incurred twice for a round trip transport. An additional Emergency Fee will be added to trips that are for emergency purposes that originate from the Wade Park Facility. Page 13, Section 4 (i), here, and in several other locations throughout this document is the reference to The Ohio Medical Transportation Board , which no longer exists. The licensing agency that needs to be referred to here is the Ohio Department of Emergency Fire and Medical Transportation Services (EFMTS). Updated information: Contractor employees performing services to VA beneficiaries under this contract must meet all requirements of Ohio Department of Transportation for drivers. Driver shall have successfully completed training in the techniques of transporting and assisting elderly and physically handicapped passengers which shall include instruction in the following elements: Discussion of characteristics of the aging process and major disabling conditions; Discussion of common assistance devices used by elderly and handicapped persons; Discussion of attitudes toward elderly and handicapped persons, which includes the participation of handicapped and elderly persons; Instruction in methods of handling gurneys; Instruction in moving, lifting and transferring passengers; Guidelines for transporting handicapped persons; Instruction in the operation of the gurney; Instruction in handling and securing all oxygen equipment on board. Each employee shall be instructed in fire extinguisher, radio dispatch, and securement of devices, and shall know what to do and who to call in a medical emergency or accident. Continuity of Services (Jan 1991) (a) The Contractor recognizes that the services under this contract are vital to the Government and must be continued without interruption and that, upon contract expiration, a successor, either the Government or another contractor, may continue them. The Contractor agrees to- (1) Furnish phase-in training; and (2) Exercise its best efforts and cooperation to effect an orderly and efficient transition to a successor. (b) The Contractor shall, upon the Contracting Officer s written notice, (1) furnish phase-in, phase-out services for up to 90 days after this contract expires and (2) negotiate in good faith a plan with a successor to determine the nature and extent of phase-in, phase-out services required. The plan shall specify a training program and a date for transferring responsibilities for each division of work described in the plan, and shall be subject to the Contracting Officer s approval. The Contractor shall provide sufficient experienced personnel during the phase-in, phase-out period to ensure that the services called for by this contract are maintained at the required level of proficiency. (c) The Contractor shall allow as many personnel as practicable to remain on the job to help the successor maintain the continuity and consistency of the services required by this contract. The Contractor also shall disclose necessary personnel records and allow the successor to conduct on-site interviews with these employees. If selected employees are agreeable to the change, the Contractor shall release them at a mutually agreeable date and negotiate transfer of their earned fringe benefits to the successor. (d) The Contractor shall be reimbursed for all reasonable phase-in, phase-out costs (i.e., costs incurred within the agreed period after contract expiration that result from phase-in, phase-out operations) and a fee (profit) not to exceed a pro rata portion of the fee (profit) under this contract. NCO 10 Contracting Office intends to release a commercial solicitation, for Ambulance services for beneficiaries of the Veterans Administration, Louis Stokes Cleveland VA Medical Center. The purpose of this sources sought notice is to seek responses from all qualified Small Business prospective offerors to include SDVOSB/VOSB verified vendors who hold a current state of Ohio Emergency Medical, Fire and Transportation Services (EMFTS) certificate to perform ambulance service in the state of Ohio to determine the type of set-aide that will be utilized for the forthcoming solicitation summarized as follows: The VA Medical Center has a policy to provide BLS/ALS transportation services for eligible veterans receiving health care in accordance with current beneficiary travel regulation and established procedures. The VA shall pay the cost of specialized mode of transportation when a VHA health care clinician determines that this mode of transport is clinically required. These transports will take place for approximately fifty (50) veterans, five to seven (5-7) days a week. The primary intent of this contract is for prearranged ambulance transportation services, with on-call services as needed and when medical emergencies arise. Contractor is to provide ambulance service on authorization from VA and administer care to patient en-route and release patient to a responsible person at destination.
