VHAPM Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT VHAPM Part 813.106 Page 2 of 2 Original Date: 08/30/17 DEPARTMENT OF VETERANS AFFAIRS Justification for Single Source Awards IAW FAR 13.106-1 For Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K) Acquisition Plan Action ID: 36C250-19-AP-3445 Contracting Activity: Department of Veterans Affairs, VISN 10, Cleveland, OH, Engineering Brief Description of Supplies/ Services required and the intended use/Estimated Amount: This activity proposes to solicit with Abbott Laboratories; a large business using sole source procurement procedures for to provide a one-year Assurance Plus Customer Protection Plan for the Ensite Velocity SN 13896667 (VA ID 1707865, 1707894) and the OPTSi Generic E.1.1.1 SN 16319205 9 (VA ID 1060964). This equipment must be serviced/repaired with the use of OEM parts, tools, and materials. The contractor must have direct access to OEM parts to ensure timely repair services and delivery. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: This request is being entered as a sole source because these items must be repaired according to manufacturer standards with the use of OEM parts, tools, and materials ONLY. This equipment type is critical to the functionality of the Cardiac Cath and EP labs and are used daily to provide direct patient care. It is essential to the operations of the Medical Center that this equipment be repaired properly and in a timely manner to ensure patient care/safety is not compromised. The Assurance Plus Customer Protection Plan shall include the following coverage terms: Extended Product Warranty: Parts Replacement Coverage I have tried to get quotes from other local companies and was unsuccessful. Description of market research conducted and results or statement why it was not conducted: FAR 13.106-1-VIP was queried under NAICS Code 811219, and no SDVOSB and VOSB contractors were identified. All repairs must be done with OEM parts, tools, materials, and software to meet specific capabilities and specifications of the above stated system. Third party vendors do not have access to OEM parts. Vendor must exhibit and be authorized the ability to complete all operational modifications according and under manufacturer specifications.
