The contractor shall furnish all necessary labor, supervision, management, management support, transportation, supplies, equipment, and materials necessary to furnish the following services. DRAFT STATEMENT OF WORK Registered Yoga Teacher for Pain Management Background: According to the Department of Veterans Affairs, Veterans Health Administration (VHA), recent data suggests that a significant percentage of veterans of Operation Iraqi Freedom (OIF)/Operation Enduring Freedom (OEF) will experience pain conditions that interfere with daily functioning and adversely affect quality of life. VHA Directive 2009-053, Pain Management, established pain management as a national priority. Scope of Objective: The contractor shall provide Registered Yoga Teacher(s) with a developed, systematic and objective approach to yoga programs specifically designed for people with pain, injuries, illness, and trauma. The contractor shall be established in assisting people in reducing chronic pain, managing injuries, preventing future injuries, and achieving a relaxed, physical, and mental state while utilizing physical movement and messages of empowerment. The contractor shall independently provide a thorough assessment of pain and functionality and determine the course of the treatment based on the assessment. The contractor shall either: independently provide either a one-hour individual and a one-hour group session of the clinically-based yoga class at the Detroit Veterans Affairs (VA) Medical Center (Detroit VAMC) for a total of two sessions each week for twelve weeks, or the veteran may be continued in one-hour individual sessions twice per week for 12 weeks or some combination thereof based on the veteran s session progress and need. The contractor shall independently provide each and every veteran enrolled in the program a one-hour individual session to make the assessment and provide patient education about the course of treatment and treatment expectations. The Veteran will be allowed a maximum of 3 individual sessions and a maximum of 12 follow up group sessions per consult. If the Veteran and the Therapist feel that more sessions are required the Therapist must consult with the Pain Management physician and or the Associate Chief of Integrated Clinical Service. The Government estimates 30 patients per year. General Information/Objectives: Contractor shall provide a customized, evidence-based, interactive yoga program with extensive participant involvement to assist in the treatment of veterans with chronic pain. Contractor shall provide a thorough assessment and session summaries regarding the veteran s treatment plan and progress. Contractor shall provide a therapeutic yoga program that will address both the physical and psychological issues that accompany living with chronic pain to help achieve total wellness. Contractor shall provide a presentation of the customized, evidence-based, individualized yoga program to hospital leadership and Pain Clinic providers with examples. Deliverables: The contractor shall provide all labor, supervision, and other resources required to facilitate the Yoga Therapy program. The contractor shall provide proof that all protocols are evidenced-based and developed with clinical expertise. The contractor shall document the evaluation of the Veterans and communicate with the referring provider. The contractor shall guide the Veterans through a series of questions designed to help them formulate personal goals on the physical, functional, emotional, and social categories. The contractor shall evaluate the Veteran on physical strength, flexibility, range of motion, balance and endurance. The contractor shall address the Veteran s specific conditions and/or injuries. The contractor shall create a specific treatment plan for each Veteran. The contractor-provided treatment plan shall create specific ways for the Veterans to build general physical health and rehabilitate the Veterans current injury or illness, while improving the Veterans mental and emotional wellbeing. The contractor shall provide the Veterans with specific activities to perform between sessions. The contractor shall guide the Veterans through the specific treatment plan created for each Veteran. The contractor shall assist the Veteran in making adjustments through the treatment plan. The contractor shall continue to communicate with the referring provider and both will evaluate the progress of the Veterans toward their goals, including changes over time in: pain level, strength, endurance, flexibility, balance, posture, psychological changes, etc. The contractor shall provide the Veteran & VA health care providers with a monthly progress report on all attendees, specifying the Veterans progress in each area, and outlining a plan for future improvements. The contractor shall provide each participating veteran one yoga props set to keep for their own use. Each set will include a Manuka Life yoga mat, 2 blocks, one strap and one bolster. Government Furnished Facility: The Detroit VAMC shall furnish office and group space for the sessions. Security: The contractor shall complete the following before being granted access to VA information and its systems: Sign and acknowledge (either manually or electronically) understanding of and responsibilities for compliance with the Contractor Rules of Behavior, Appendix E relating to access to VA information and information systems; Successfully complete the VA Privacy and Information Security Awareness and Rules of Behavior training and annually complete required security training; Successfully complete the VA 10203 Privacy and HIPAA training in TMS. Annually complete required privacy and Information Security and Rules of Behavior training. Place of Performance: John D.
