This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-06. The associated North American Industrial Classification System (NAICS) code for this procurement is 541380, with a small business size standard of $15 million. The Dayton, Ohio VAMC is seeking to purchase occupational dosimetry services. Statement of Work: Deliverables The vendor shall provide radiation dosimeters and perform analysis of radiation dosimeters upon return to the vendor and provide a record of occupational radiation exposure. The radiation dosimeters shall be worn by monitored individuals and returned to the vendor after the designated wear period for analysis. The vendor shall supply various reports to the Radiation Safety Officer (RSO). The vendor shall provide an on-line, remote, radiation safety training to employees of the Dayton VA Medical Center. Radiation Dosimeters The vendor shall provide passive dosimeters that accumulate/store dose until it is shipped to the vendor for processing and measuring dose (reading). The following types of dosimeter shall be supplied: Whole-body (x-ray, gamma ray, beta) Whole-body (x-ray, gamma ray, beta, neutron) Extremity (x-ray, gamma ray, beta) Lens of Eye (x-ray, gamma ray, beta) Environmental (x-ray, gamma ray, beta, neutron) The dosimeter shall be composed of either a thermoluminescent material (TL) or an optically stimulated luminescent material (OSL). The whole-body dosimeters used to detect x-ray and gamma radiation shall be capable of dose measurement as low as one (1) mrem. The whole-body dosimeters used to detect neutron radiation shall be capable of dose measurements as low as ten (10) mrem for thermal neutrons and twenty (20) mrem for fast neutrons. The extremity dosimeters used to detect x-ray and gamma radiation shall be capable of dose measurement as low as ten (10) mrem. The lens dose dosimeters used to detect x-ray and gamma radiation shall be capable of dose measurement as low as one (10) mrem Dosimeters shall be color coded to identify the wear period and have distinct markings to aid in proper wear positioning. Dosimeters shall be durable, waterproof, and be labeled with the dosimeter series, dosimeter type, monitored individuals name and wear period. Extremity dosimeters (rings) shall be available in at least three (3) sizes. Vendor shall provide option to calculate and report Effective Dose Equivalent (EDE) based on wear of either one or two dosimeters for personnel routinely wearing a lead apron/vest. Vendor shall provide a web-based account management system for use by the RSO. Dosimeters shall be shipped to the RSO and shall arrive at least one week prior to the next wear period. The wear period be monthly or quarterly as stipulated by the RSO. Wear periods shall start on the 1st of the month. Radiation dosimeters shall be shipped to the RSO in presorted envelopes/package labeled for each subaccount. Vendor shall provide emergency processing of dosimeter when requested. This processing shall be completed within 24 hours of receipt of the dosimeter by the vendor. The radiation safety officer or designee shall be notified of the results by telephone or e-mail within 24 hours of receipt of the dosimeter by the vendor. Vendor shall provide pre-paid shipping package (e.g., envelope, box) for returning dosimeters for processing. Reports The vendor shall provide reports to the RSO. The following are required: Dose Summary Reports - Vendor shall provide monthly/quarterly summary reports for each subaccount within 20 calendar days of receipt of the dosimeters. One copy of the report shall omit sensitive employee information including; social security number and birth date to allow posting in work areas. The other copy of the report shall include social security number and/or date of birth. Fetal Dose Reports - Vendor shall provide monthly report(s) of the fetal dose each monitored pregnant worker. Annual Occupational Dose Reports Vendor shall provide annual dose reports (i.e., US NRC Form 5 or equivalent) for each monitored worker per US NRC Regulatory Guide, 8.7, Instructions for Recording and Reporting Occupational Radiation Dose Data. Reports shall be provided to the RSO prior to May 1st each year. Vendor shall also provide archival quality CD-ROM/DVD containing all annual dose reports for all monitored workers each year. Termination Report Summary Vendor shall supply dose report for within 45 days of receipt of dosimeter and disenrollment from the occupational dosimetry program. Unreturned or Damaged Dosimeter Report Vendor shall provide a report of all missing or damaged dosimeters. Training The vendor shall provide on-line training for Dayton VA Medical Center personnel for the following: General Radiation Safety CT Radiation Safety, Dose and Risk for Medical and Dental Practitioners, Technologists and Staff Radiation Dose Optimization and the Safe Use of CT: Image Gently®, Image Wisely® Radiation Safety Training Fluoroscopy Users - Cardiac Labs, Intervention Labs, Mobile C-arms Training Customer Service The vendor shall be accessible via telephone, e-mail, or electronic messaging during regular working hours and ensure timely response to RSO inquiries. The Contracting Officer shall not negotiate with any quoters other than those that represent the best value to the Government and shall not use the formal source selection procedures described in FAR Part 15. The Contracting Officer may issue a purchase order to other than the lowest priced quoter. The Government may award a contract resulting from this solicitation to the responsible quoter whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes: Price: The Vendor shall provide one electronic copy of quote Past Performance: Provide (3) references of work, similar in scope and size with the requirement detailed in the Description and Scope. References shall include contact information; brief description of the work completed, and contract # (if relevant). References may be checked by the Contracting Officer to ensure your company is capable of performing the required services. The Government also reserves the right to obtain information for use in the evaluation of past performance from any and all sources to include PPIRS. Technical: The vendor s quote shall be evaluated to determine if the organization has the experience and capabilities to provide the requested services in accordance with the Statement of Work in a timely efficient manner. The vendor shall provide the following to assist with technical evaluation: Provide proof of NVLAP accreditation. Provide written specifications for dosimeters to include: images of each dosimeter minimum detectable dose, energy discrimination capabilities including accuracy, precision, and remeasurement for the differentiation of static versus dynamic exposure maximum range of reported dose for the various energy ranges concerned, primarily: Less than 100 keV photons 100 keV through 250 keV photons Greater than 250 keV up to and including Cs-137 and Co-60 Photons from a 6/10/15/18 MV medical linear accelerator Beta radiation Neutrons (fast and thermal) Provide information regarding procedures for management of the dosimetry program in terms of describing how the Radiation Safety Officer (RSO): Adds new personnel. Deletes personnel. Transfers personnel to different subaccount. Assigns a spare dosimeter. Orders dosimeters for newly enrolled personnel to arrive before next dosimeter exchange. Assign a fetal dosimeter. Administratively assign dose in the event a dosimeter is not returned or damaged. Retrieves historical dosimetry data on an account, a subaccount, or an individual basis. Provide information regarding permanent archival of dosimetry results and method of access. Provide description of on-line radiation safety training program in terms of describing: How individuals enrolled in the personnel dosimetry program access the training. How individuals are notified of requirement to complete training. How records of completion of training are provided. The topics covered in each training module and estimated time to complete each module. If completion of the training module provides continuing education credit and if so how many hours. This is an open-market combined synopsis/solicitation for services as defined herein.  The government intends to award a purchase order as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes shall include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" Quoter shall list exception(s) and rationale for the exception(s). Submission shall be received not later than June 12, 2020 at 4 PM to maryjane.crim@va.gov. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f).
