In accordance with FAR Part 10, Market Research, this is a sources sought to determine the availability of potential sources/vendors having the skills and capabilities necessary to provide 2.8 FTE experienced (minimum 2 years of experience) Nurse Practitioner (NP) to perform Medical Officer of the Day (MOD) services to the Department of Veterans Affairs, Northern Indiana Health Care System (VANIHCS), onsite at 1700 E. 38th St., Marion, IN 46953. All interested contractors are invited to provide information to contribute to this market survey/sources sought including, commercial market information. This sources sought notice is for information and planning purposes only and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). Period of performance: 9/25/2020 through 9/24/2021, plus the possibility of four option years, 5,856 hours annually. Hours of service will be 7:00pm through 7:00am Monday through Friday; 7:00pm Friday through 7:00am Monday (entire weekend); and 24-hour coverage on federal holidays. The Nurse Practitioner MOD will be the sole provider on duty during the prescribed hours of coverage and shall have experience with providing care to residents who need geriatric, medical, and/or mental health care. The NP and must have a current license to practice as a nurse practitioner in any State, Territory, or Commonwealth of the Unites States or the District of Columbia. The contractor shall propose adequate key personnel/FTEE to be credentialed, privileged, and be available for scheduling to meet the requirements of the resultant contract, taking into account sick leave, vacation, or nurses practitioner(s) leaves Contractor s employment or is unable to continue performance in accordance with the terms and conditions. Capabilities/Qualifications: A written response providing clear and unambiguous evidence to substantiate the capacity to fulfill this requirement. Description of the capabilities/qualifications/skills your company possesses to perform services described. Do you need to subcontract any portion of work to provide these services stated herein? Past Experience relative to similar requirements. Offer may provide the following information on similar projects for which the responder was a prime contractor. The name, address, and value of each project. The Prime Contract Type, Firm Fixed-Price. The name, telephone and address of the owner of each project. A description of each project and why it is relevant to this requirement. Include difficulties and successes. Your company s role and services provided for each project.
