This Sources Sought/Market Survey Notice is being conducted by Network Contracting Office 10 (NCO 10), PO Box 492, Ann Arbor, MI 48106 to identify other sources that can provide Skilled Nursing Home Services in the following counties in Indian: Wabash and Allen to beneficiaries of the Northern Indiana VA Medical Center, with locations in Marion and Fort Wayne, Indiana. Period of Performance: two one-year ordering periods from on or about February 15, 2020 to February 14, 2022.
