This announcement constitutes the only solicitation; quotations are being requested and a written solicitation document will not be issued. This solicitation is issued as a Request for Quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2020-01, effective November 9, 2019. This solicitation is a 100% small business set aside and only qualified contractors may submit quotes. This requirement will be awarded on all-or-none basis. The Government will issue a Firm-Fixed Price, brand name only purchase order resulting from this solicitation. Statement of Work: PURPOSE: The purpose of this Statement of Work (SOW) is to develop a firm fixed price purchase order to be used by Cleveland Veterans Affairs Medical Center (CLE VAMC) for the purchase of a Hausted Motorized Eye Carts as a direct replacement/upgrade to existing equipment. SCOPE OF SERVICE: Contractor shall provide five (5) motorized eye stretcher carts with contoured head rests and two (2) with wrist rests, with any required assembly and staff education as needed. OBJECTIVE: Motorized eye stretcher carts will be utilized in Outpatient Ambulatory Surgery for patients that need specialize eye procedures that require precise movement of the head piece and body positioning to prevent injury to patient, ensure proper body mechanics of caregiver and allow for ease of use for physician/anesthesia during surgery. REQUIREMENTS/TASKS: This is a brand name only solicitation. Eye carts shall be Hausted Surgi-Stretcher electric/battery powered model 5E8EYEST. Salient characteristics as follows: Shall have 3 inch wipeable mattress pad and head pillow Head rest pillow shall be contoured Head rest shall be removable Motorized eye stretcher carts shall measure minimum 24 wide Carts shall have motorized head and foot controls by use of remote as well as controls for head and foot maneuverability located on the stretcher eye cart itself Shall provide the ability to move patient to Trendelenburg or reverse Trendelenburg position via a remote control with optional controls for positioning placed on the eye cart as well. There shall be manual maneuverability controls to place patient in Trendelenburg in the event of emergency and the electronic remote and bed panel do not function. Shall have adjustable precision head controls to move patients head during surgery. Shall have optional U-shaped wrist support that can be added, when needed, by the surgeon during surgery. Shall have holder to transport oxygen on the stretcher. Shall have a minimum of a 1 year Warranty Shall include Permanent IV Pole Shall include a battery backup system Minimum weight capacity shall be 550 lbs. Offer Evaluation and Award: This is a competitive Request for Quotations (RFQ) that will be evaluated under FAR Part 12 and 13. The Government will issue a Firm-Fixed Price Purchase Order resulting from this solicitation to the responsible offeror whose offer conforms to the requirements within this solicitation and will be the most advantageous to the Government. Best Value will be how the quotes are evaluated. The following factors will be used in evaluation: A price comparison along with Technical Acceptability, Delivery, and Warranty. An Offeror s initial quote shall contain the best terms from a price and technical standpoint. Offerors are reminded that representatives from your company SHALL NOT contact any Cleveland VAMC personnel to discuss this RFQ during the solicitation and evaluation process. 6. Procedure for submitting documents to the Department of Veterans Affairs. All information shall be submitted in the manner described below and offerors who do not submit all information may not be considered for award. This requirement is brand name only. Contractor shall provide an authorized distribution letter. The Contractor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor, or authorized Contractor reseller for the proposed equipment/system such that the OEM warranty and service are provided and maintained by the OEM. All warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. The Contractor shall provide new items ONLY; no remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. Quote must be good for 90 calendar days after close of this Combined Synopsis Solicitation.
