The STERILIZER MAINTENANCE shall be performed for the ALEDA E. LUTZ VA MEDICAL CENTER, 1500 WEISS STREET, SAGINAW, MI 48602 Contractors that can provide this requirement and meets the requirements below are requested to send their 1. Company Name, 2. DUNS number, 3. Business size under NAICS 811219 and 4. If the item(s) are available on an FSS contract or not and the FSS contract number if it is available on contract, 5. Notification if they are a VETBIZ registered VOSB/SDVOSB, SMALL BUSINESS, WOMEN OWNED SMALL BUSINESS, COMPETETIVE 8 (a), or not. 52.219-14 Limitations on Subcontracting. Services, At least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern. This is not a solicitation. INTERESTED CONTRACTORS SHALL SUBMIT WRITTEN DOCUMENTATION SHOWING THEIR ABILITY TO COMPLETE THE REQUIRED STERILIZER MAINTENANCE. B.2 Statement of Work (SOW) Sterilizer Maintenance STATEMENT OF WORK 1. SCOPE: Contractor shall furnish all labor, transportation, parts, and expertise necessary to provide calibration, preventive maintenance, and unlimited emergency on-site corrective maintenance visits to all government-owned equipment at the ALEDA E. LUTZ VA MEDICAL CENTER as identified on the attached equipment list (Attachment 1 listed below). This contract will be a base year with four (4) one (1) year options. Place of performance is the ALEDA E. LUTZ VA MEDICAL CENTER, 1500 WEISS STREET, SAGINAW, MI 48602. 2. QUALIFICATIONS: To be considered eligible for consideration, offerors shall have a field service technician that has been trained by the Original Equipment Manufacturer (OEM) on the types of equipment the technician is being asked to work on. Vendor shall provide documentation of factory certified service/maintenance training on the types of equipment under the terms of this contract. Contractor shall also provide points of contact at three other medical centers with which they are or have been contracted to service equipment of the same or similar sterilizer equipment models as those listed in Attachment 1 in the past three years. Contractor shall be able to provide 24-hour physical on-site response time to urgently repair critical SPS equipment. Subcontracting of any ensuing award of this solicitation shall not be allowed. 3. SERVICES TO BE PROVIDED: A. General: 1) Contractor shall provide the necessary manpower and supervision to properly execute the maintenance and repair of the equipment listed in Attachment 1. The contractor shall provide OEM parts for any repairs and preventative maintenance of equipment in Attachment 1. Any repair parts with a cost of $100.00 or less shall be purchased by the contractor. The contractor shall supply a written detailed quote to the COR for any parts over the $100.00 cost. The contractor cannot proceed with the additional repair until an authorized purchase order is issued. 2) A Field Service Report (FSR) shall be generated and a copy given to the ALEDA E. LUTZ VA MEDICAL CENTER Contracting Officer s Representative (COR) or designee after each repair needed under a normal ALEDA E. LUTZ VA MEDICAL CENTER repair Work Order or emergency maintenance. It is preferred that this FSR be provided electronically if possible. 3) All mobile media (ie flash drives, CD/DVD disks, etc.) that are required to complete the repairs on the equipment listed in Attachment 1 shall be scanned for viruses prior to being connected to Aleda E. Lutz VA Medical Center equipment. If the vendor must connect a vendor-owned laptop to perform the repairs, they shall show proof that the laptop has current anti-virus software and that a recent scan shows no infections on the laptop. B. Repair: 1) All none emergency repairs shall be performed during normal VA business hours, 6:00am to 4:30pm, Monday thru Friday, except Federal Holidays or any Federal Holiday as authorized by the President of the United States which include: New Year's Day, Martin Luther King, Jr. Birthday, Washington's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day 2) If work must be performed outside of normal business hours ( over-time work), the contractor shall obtain prior approval from the COR. If such work is at the cost of the ALEDA E. LUTZ VA MEDICAL CENTER, a purchase order number shall be obtained by the contractor prior to performing any over-time work. 3) If the contractor s telephones are not answered by technical personnel, a technical maintenance representative shall respond by telephone to the VA within 4 hours of a service call from the VA. The contractor shall arrive on-site for repair within 24 hours of placing a service call by the VA. 4) Contractor shall furnish all tools and materials (e.g. service manuals, diagnostic software, etc.) required to maintain the equipment in top operating condition. Any precision tools or diagnostic instruments used for repairs shall have a yearly calibration test with certification. These certifications shall be presented to the COR at the beginning of each contract year or as requested by the COR. 5) Any software updates with unlimited phone support for the equipment software shall be included. C. Preventive Maintenance: 1) Preventive Maintenance shall be performed by the contractor at the intervals that are in accordance with the manufacturer s recommendations, but no less than annually. 2) Contractor shall furnish any required materials (PM kits), manuals, tools, etc. needed to perform Preventive Maintenance. 3) The Contractor shall perform preventive maintenance according to manufacturer s specifications. The Contractor shall have written procedures to be followed and documented evidence that each piece of equipment has been inspected according to those procedures. 4) The Contractor shall on a yearly schedule, starting within the first quarter of the contact year include the cleaning of the sterilizer chambers on equipment: Asset Description Mfg. Documentation: A documentation package acceptable for Joint Commission purposes shall be maintained by the contractor and sent electronically to the COR within 48 hours upon completion of the work. Required features include, at a minimum: 1) Date of service 2) Equipment Model 3) Equipment Serial Number 4) Location (Building/Room Number) 5) Service Representative Name 6) Any and all work performed, including parts replaced E. New Equipment: If new equipment or components are obtained, either through new purchase, upgrade or replacement of existing equipment, that equipment may be added (and the replaced equipment deleted) from the service contract as needed upon notification of the Contracting Officer (to be documented in writing via contract modification). Deleted equipment shall be credited in the full amount if deleted before any maintenance or repair has been performed on it. F. SPECIAL INSTRUCTIONS: 1) Contractor Check-In: The contractor s representative shall contact Engineering at (313) 576-1000, ext. 61219, prior to performing service at the ALEDA E. LUTZ VA MEDICAL CENTR. 2) Documentation: At the conclusion of each repair and scheduled maintenance visit, the contractor shall provide a written service report indicating the date of service, the model, serial number, and location of equipment serviced; the name of the representative, and the services performed and parts replaced.
