This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this RFI is to obtain knowledge and information for acquisition planning purposes. The Department of Veterans Affairs Network 10 Contracting Office is conducting market research to identify potential Small Business sources that meet the requirements generally described in this notice. The intent of this announcement is to: Notify potential sources about the requirement Identify potential Small Business sources Meet agency s small business goals under 8(a), HUBZone, SDVOSB or WOSB programs. FAR 19.5 The Department of Veterans Affairs, Network 10 Contracting Office, on behalf of the Chillicothe VA Medical Center is seeking to acquire service and maintenance of Belimed Sterile Processing Equipment located at 17273 State Route 104, Chillicothe, OH 45601. The Government wishes to have a plan for a base year plus four (4) option years for a total of five years of service. Information Requested Vendors interested in this Sources Sought Notice may provide a response that includes information on your firm s capability to provide the required services, as written in the attached DRAFT Statement of Work. Please also provide your firm s DUNS number, organization name, address, business size and type as validated via the Central Contractor Registry (CCR) (e.g., 8(a), WOSB, VOSB, SDVOB, or HUBZone small business) pursuant to the applicable NAICS code. Respondent s points of contact, including names, titles, addresses, telephone and fax numbers, and e-mail addresses. Please note the DRAFT Statement of Work attached to this announcement is not in its final form. The Government reserves the right, through internal acquisition procedures prior to the release of the formal Request for Quotes (RFQ), revise the Statement of Work to better reflect the Government s requirements. Instructions for Submittal Written responses shall be provided electronically, by 11:00 a.m. EST on May 13, 2019. The subject line of the e-mail message shall read: Belimed Sterile Processing Equipment Services Sources Sought Response and shall be addressed to the attention of: starla.vanwinkle@va.gov. The VA will not accept telephone, facsimile, courier, mail, or hand delivery methods of response.
