This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation RFQ-36C25019Q0673 is issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-94, 2005-95/ 01-19-2017. The North American Industry Classification System (NAICS) number is 339113 and the business size standard is 750 employees. BRAND NAME OR EQUIVALENT Description Quantity Wheelchair Bike 3 Removeable Raised Armrests 3 Hand Shield 6 The VA Northern Indiana Health care system has a need for a tandem wheelchair bike at its Marion Facility with the following specifications: Bike shall have a wheelchair in the front capable of seating a patient Wheelchair shall have seat belt to maintain position of patient during operation. Wheelchair shall have arm rests and hand guards to prevent patient from contacting wheels while in motion Bike section shall be positioned behind the wheelchair and shall be attached in a secure and safe manner. Bike section shall have a bicycle seat and pedals for the operator to propel the bike with a patient in the wheelchair, so the operator can see the path ahead and maintain view of the patient. Bike pedal assembly shall have at least three speeds, and adequate brakes to stop the vehicle effectively with patient in the wheelchair. The entire vehicle shall be steerable by the operator only Bike section shall be detachable so as to allow the wheelchair to be used separately for ease in loading and unloading patients Total weight capacity for passenger and operator minimum 550 lbs. Total wheelchair weight capacity minimum 275 lbs. The vendor will ship the equipment to the VA Marion Healthcare System, 1700 East 38th Street, Marion, IN 46953 The proposed contract listed here is Set-aside 100% for SDVOSB. FOB destination.
