This Sources Sought/Market Survey Notice is being conducted by Network Contracting Office (NCO) 10, PO Box 492, Ann Arbor, Michigan 48106 to identify other sources that are capable of providing residential treatment services as part the Healthcare for Homeless Veterans Program for homeless veterans, male or female who are beneficiaries of the Battle Creek VAMC. The contractor shall provide housing and supporting services (case management, permanent housing search, recovery services) to Homeless Veterans with or without their families in community-based Contracted Residential Services (CRS) facilities offering a safe and secure environment that supports their goals for recovery from homelessness. The requirement is for a facility with 18 beds. Facility to be located within the city limits of Grand Rapids, Michigan. Firm fixed, IDIQ contract with Five (5) One-Year ordering periods, starting August 8, 2019.
