Answer: The brand name for CLIN 0001 is Performance Mat s Performance Fall Mat. The brand name for CLIN 0002 is Performance Health s Fall Prevention Mat. Please refer to the Salient Characteristics document found on Page 3 of the original solicitation for the Minimum requirements for equal products. This is a BRAND NAME OR EQUAL combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-05. This solicitation is set-aside for Small Business. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112, with a small business size standard of 1,000 employees. The FSC/PSC is 6530. The James H. Quillen VA Medical Center is seeking to purchase Fall Mats. All interested companies shall provide quotations for the following: a. Only one quote. Quote shall be on offeror s company letterhead, in PDF format and emailed to jonathan.headrick@va.gov NLT 11/12/2024 at 9:00 AM ET. Questions are due NLT 11/07/2024 9:00 AM ET and must be sent to jonathan.headrick@va.gov. The solicitation number 36C24925Q0052 must be referenced in the subject line. b. Product information brochure with specifications and user manual shall accompany quote. c. Offeror must submit certification that equipment meets the salient characteristics listed. d. Quote must reflect anticipated delivery date after receipt of order. e. The Line Items must be completed with product information and submitted along with offer. f. Proof of Small Business status must be included with offer. Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. The following are the decision factors: (1) Conformance to Salient Characteristics (2) Technical Factors (3) Price Factors The award will be made to the response most advantageous to the Government.
