Page 1 of 1 version 0001 Dated 05/02/2016 SOURCES SOUGHT The purpose of this Sources Sought Notice is to search for contractors capable of providing Community Based Outpatient Clinic (CBOC) Services for the Department of Veterans Affairs, Memphis TN. The place of performance will be Tupelo Mississippi (Lee County) within 20 miles of a local hospital providing emergency room services. Period of Performance: BASE PERIOD April 1, 2026 through March 31, 2027 plus (9) one year option periods. This Sources Sought Notice is for market research purposes only and shall not be considered an Invitation for Bids, Request for Quote, or a Request for Proposal. Do not submit a proposal or quote in response to this notice. There is no obligation on the part of the Government to acquire any products or services described in this sources sought notice. You will not be entitled to payment for direct or indirect costs that you incur in responding and the Government does not intend to pay for the information submitted in response to this notice. Interested parties are responsible for adequately marking proprietary, restricted, or otherwise sensitive information contained in their response. Any interested business concerns with the capability to provide services must submit a capability statement addressing its ability to meet the requirements in the attached draft Performance Work Statement. Capability statements shall also include: Name, address, phone number, email address, and point of contact for the company. System for Award Management Unique Entity ID (UEI) number. Business size and NAICS code. Capability statements shall be sent to Dana Dixon, Contract Specialist, at dana.dixon@va.gov no later than 4:00 pm Central on July 26, 2024.
