A.5 SCOPE OF WORK Project Title: Replace ED Flooring Project Location: Troy Bowling Campus Lexington VAMC, Lexington, Kentucky, Main Building, 1st Floor Project Number: 596A4-19-108 Scope of Work: Contractor shall completely prepare site at the Troy Bowling Campus of Lexington VAMC in Lexington, Kentucky including demolition and removal of existing doors and flooring in the Emergency Department. Contractor shall furnish all labor and materials and equipment to perform work for installation of doors and flooring per furnished drawings and specifications at the Troy Bowling Campus of Lexington VAMC, Lexington, Kentucky. JUSTIFICATION: This project is required to replace damaged and deteriorating flooring and doors in the ED at Troy Bowling campus. Current ED flooring has holes and cracks throughout. Floor should be replaced to avoid injury to patients and staff.

SET-ASIDE: 100% Service-Disabled Veteran-Owned Small Business (SDVOSB) PROJECT MAGNITUDE: Between $250,000.00 and $500,000.00 NAICS: 236220 Commercial and Institutional Building Construction SMALL BUSINESS SIZE STANDARD: $39.5 Million PROJECT OVERVIEW: The project will require the selected contractor to provide all resources necessary to accomplish and furnish all labor, material, equipment, transportation, and supervision necessary to completely prepare site for building operations as required per drawings and specifications at the VA Medical Center Lexington 1101 Veterans Drive Lexington, KY 40502-2236 for the above-mentioned Project # 596A4-19-108 Replace ED Flooring located at Troy Bowling Campus. Anticipated completion time from Notice-to-Proceed is 215 calendar days for the entire project.
