The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Background: The Contractor shall furnish all necessary labor, supervision, equipment, and material to perform preventive maintenance, inspecting, certifying, and on-site testing of the fire alarm and sprinkler systems to meet NFPA standards. This work is for the Lexington VAMC Franklin R. Sousley Campus located at 2250 Leestown Rd, Lexington, KY. Service shall be completed during the normal business hours of 8:00 a.m. 4:30 p.m Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 541990 and sized standard is $17.0M. NOTE: Respondent claiming SDVOSB and VOSB status must be registered and CVE verified in VetBiz Registry www.vetbiz.gov. Statement of Work PART A GENERAL INFORMATION The Contractor shall furnish all necessary labor, supervision, equipment, and material to perform preventive maintenance, inspecting, certifying, and on-site testing of the fire alarm and sprinkler systems to meet NFPA standards. This work is for the Lexington VAMC Franklin R. Sousley Campus located at 2250 Leestown Rd, Lexington, KY. Service shall be completed during the normal business hours of 8:00 a.m. 4:30 p.m. ET, (unless other times are stipulated by the Contracting Officer s Representative (COR)). PART B WORK REQUIREMENTS B.1 Requirements: All tests shall be evaluated against the NFPA code which requires having maintenance testing and record keeping programs in place. At the completion of the inspections, a professionally bound certification report in triplicate, which includes detailed existence, placement and performance results and maintenance requirements of each component of the system, shall be sent to the COR (Contracting Officer s Representative). The test results shall be sent to VA COR representatives within 14 business days. The following are the lists of NFPA codes that shall be followed with the most current editions. NFPA 72, Table 14.4.3.2, NFPA 25, 13.2.5, NFPA 96, 11.2.1, NFPA 10, 7.2 & 7.3, NFPA 90A, 6.4.1. All work shall be completed in accordance with local, State and Federal regulations (including the local Fire Marshal). Contractor shall provide daily cleanup of work area and trash removal from site. All tests shall be performed by certified technician(s). All inspections shall comply with applicable NFPA standards and include a full inspection report with all corrective actions needed for compliance. B.2 Tasks to be accomplished: Quarterly inspections and certification of the Hospital s sprinkler system s main drains and Fire dept connections. (18 Main Drains/56 Riser Drains, 18 FDC s) (The Joint Commission EP9 and EP10) Semi-Annual inspections and certification of fire alarm tamper switches and water flow devices. (96 Tampers, 99 Waterflows) (TJC EP1 and EP2) Annual inspections and certifications on five (5) Backflow Prevention devices connected to the municipal water supply at the Sousley Campus. Three (3) are domestic water and two (2) are fire. Devices will be tested for proper installation and operation. Service will be completed in the month of June. Annual test and inspection of Fire Hydrants. (14 each) 5 Year Standpipe inspection and certification due August 2025. (TJC EP12) Monthly inspections and certification of all Fire Extinguishers. (265 each) (TJC EP15) Annual Maintenance of all Fire Extinguishers. (265 each) (TJC EP16) Annual inspection and testing of door assemblies by qualified person. (182 each) (TJC EP25) Annual inspections and certification of the Hospital s fire alarm system and all associated devices. Twice a year, an inspection shall be done on 50% of devices. At the end of the year, 100% of the fire alarm system shall be completed. (1793 total devices) (TJC EP1-5, EP19 and LS EP7) Semi-Annual visual only inspections of the Hospital s fire alarm system and all associated devices. Twice a year, a visual inspection shall be done on 50% of devices. At the end of the year, 100% of the fire alarm system shall be completed. (1793 total devices) Annual inspections of main fire alarm panels, Notifier NFS2-640, all associated power supplies and batteries. PART C SUPPORTING INFORMATION C.1 Contractor Furnished Materials All labor, materials, equipment to perform the job. When "furnish", "provide", "install", or similar term is used it shall mean a complete installation, ready for use. C.2 Government Furnished Materials and Services. Government furnished property, if any, will be identified and will only be used for the performance of this contract unless otherwise authorized by the Contracting Officer. The Contractor shall always take all steps necessary for maintenance and preservation of all Government-furnished property. The Contractor shall comply with all reasonable requests of the Contracting Officer to enclose, or specifically protect, Government-furnished property. C.3 Qualifications of Key Personnel Contractor s employees shall be knowledgeable of standards associated with this type of work. C.4 Supervision/Communications. At all times during the performance, the Contractor's Superintendent (if applicable) shall be available by cellular phone. Prior to beginning any work, the Contractor shall supply the COR with the telephone number for the Superintendent. The Contractor's "Competent Person" that meets OSHA training guidelines, who can make decisions and acting on behalf of the Contractor, shall be physically always located on site during performance of the contract. If the Contractor's Competent Person is absent from the job site for an extended period of time, the COR may send all Contractor/sub-contractor employees off the job. C.5 Security Requirements: Contractor shall identify which employees will be working under the awarded contract. Contractor personnel shall include employees, sub-contractors, subcontractor employees, suppliers and delivery personnel entering the Medical Center. This does not apply to suppliers and delivery personnel making deliveries to the VA warehouse dock only. Those employees shall bring photo ID to obtain a VA Contractor identification badge that shall be worn above the belt at all times while on Medical Center property. Contractor's employees shall return all I.D. badges to designated personnel. C.6 The Contracting Officer's Representative (COR) is responsible for the inspection of the work called for in this contract and will be identified at the time of award. Inspections made by the COR are for the sole benefit of the Government and does not relieve the Contractor of any quality control responsibilities. C.7 The Safety Manger is responsible for safety inspections of all contract operations. C.8 Drawings General. Contract drawings (if applicable) will indicate the extent and location of work. If any departure from the contract drawings is deemed necessary by the Contractor, the details of such departure and the reasons therefore shall be submitted to the Contracting Officer for approval as soon as practical. No such departure shall be made without proper written permission of the Contracting Officer. The Contractor is responsible for all information and requirements in drawings and specifications that pertain to the contract. C.9 Verification of Dimensions The Contractor shall visit the job site to thoroughly familiarize himself with all the details of the work and working conditions. The Contractor shall also verify all dimensions in the field and shall advise the Contracting Officer of any discrepancy before performing any work. The Contractor shall be specifically responsible for the coordination and proper relation of his work to the building, structure, phasing and ensure the safety of employees and workmen. C.10 Smoking Policy Smoking is prohibited on federal property. C.11 Parking/Traffic Regulations - The Contractor shall not park on grassy areas unless approved by the COR and the Contractor agrees to restore areas back to VA standards. Drivers shall be particularly concerned with pedestrian traffic and yield to pedestrians in crosswalks. Posted speed limits and all other traffic controls are to be observed by operators at all times. Seat belt use is mandatory on the VA grounds. D.
