DEPARTMENT OF VETERANS AFFAIRS Robley Rex VA Medical Center, Louisville, Kentucky STATEMENT OF WORK Wayfinding Signage Touchscreens REQUEST FOR QUOTATION: The intent of this Request for Quotation is to establish a contract for installation of wayfinding signage located at the Robley Rex VA Medical Center in Louisville, Kentucky. Contract to includes all labor, material, parts, and equipment. All work will meet given specification and be fully operational according to V.A. working performance standards. All work shall be performed in accordance with the terms and conditions contained herein INTRODUCTION This Statement of Work describes the requirements for the work done at the Robley Rex VA Medical Center Contractor shall install equal or like digital signage system as described below. Number of digital, wayfinding signs to be installed (requested 8), Digital System to include head end server. Project must meet ANSI, OSHA, and VA regulation VHA 2009-026. Digital Signage needs to be installed in preselected areas that will be selected by the VA Medical Media staff and VA Engineering staff after site survey is completed. Wayfinding Signage will be installed at listed locations and by listed priority. Definition of Wayfinding: information systems that guide people through a physical environment and enhance their understanding and experience of the space. Wayfinding is important in complex-built environments such as, healthcare and educational campuses. As architectural environments become more complicated, people need visual cues such as maps, directions, and symbols to help guide them to their destinations. In these often high-stress environments, effective wayfinding systems contribute to a sense of well-being, safety, and security. The Contractor PM will be the single point of contact for VAMC on the project. The PM will mobilize contractor resources to complete the following tasks: Coordinate kick-off meeting between contractor and VAMC Order and ship the equipment Assign an installation service manager for the project The contractor/PM to perform site survey as well as installation. The outcome of the site survey is to determine scope of work and timeframe to complete the job. During the site visit, it shall be determining the appropriate locations for mounting. For installation, contractor will install the Pantella surface-mounts, digital media player; . Professional installation includes installing with wall mount at sheetrock wall with Media Player compartment and necessary cabling (up to 100' of Ethernet cable, up to 6' of HDMI cable), and USB to Serial adapter. Once installation is complete, Contractor will provide approved background & template designs, network setup, configuration, testing, and turn up. Contractor shall provide next business day (NBD) onsite Field Maintenance as well as software maintenance for a period of three years. Contractor shall ensure the walls where the TV s will be mounted at are strong enough to handle the TV s. In addition. Contractor will remove existing touchscreens and return units to VA Medical Media staff, bldg. 4, room 4102 at 800 Zorn Avenue. VA will provide data and electric drops to locations. All electric work will be completed in accordance with NEC and VA regulations. Contractor shall supply and install equal or like equipment, plus functions and services listed below: Continued: Contractor will be required to complete installations including network setup, configuration, testing, and turn up to all locations receiving wayfinding signage at Robley Rex VAMC, bldg. 1, locations: Basement @ Clinic Elevators x 1 Basement @ Main Elevators x 1 First floor @ Clinic Elevators x 1 First floor @ Main Elevators x 1 First floor @ Clinic/ED Entrance x 1 Second floor @ Clinic Elevators x 1 Second floor @ Main Elevators x 1 Third floor @ Main Elevators x 1 All product surfaces shall be free of scratches, mars, dents, or blemishes. No flacking, cracking and/or inconsistent coloration. Monitors to be calibrated upon installation. The project includes any incidental piping, connections, electricity, and any other associated utilities. Patching and repairing of any walls effected during installation. Work shall be performed during normal working hours and the schedule must be approved by the POC prior to work start. Contractor is responsible for removal of all packing material, cardboard, plastic wrap, etc. Site to be left in a clean presentable manner. GENERAL INSTRUCTIONS: The Contractor may be subject to the application of VA Directive 0710 titled Personnel Security and Suitability Program from June 4, 2010. The Contractor shall provide all necessary supplies, materials, equipment, labor, testing, certification, expert supervision, management, and transportation to satisfactorily perform all tasks as identified within. The number of working days to complete the project is 30 working days after issuance of the notice to proceed. All work will be in accordance with the latest VA, OSHA and physical security design guides, specifications, directives, standards and requirements, energy conservation methods, NEC, NFPA, Life Safety Code, VA Infection Control Standards, and applicable local and federal codes and/or regulations. At completion of work, all surfaces shall be left clean without any sign of left-over paint, stains, or debris. The Contractor shall keep the POC and the Contracting Officer informed at all times of the status and progress of the project. The contractor may be required to go thru General Hospital Environment Awareness information and/or training. The contractor will coordinate with owner before disconnecting any circuit or conducting any utilities modification or stoppage. Any shutdown shall be coordinated with COTR and Engineering Service. SPECIFIC INSTRUCTIONS: The Contractor will perform a survey of the existing facilities and perform an actual assessment of the existing areas. The walk thru will include obtaining any required information. The contractor will patch, float, tape, sand, and paint to existing all areas affected by his construction operations. The contractor shall fully comply with all infection control requirements. At a minimum, the contractor shall do the following actions: Use mobile containment unit for dust control if possible. Replace ceiling tile(s) as soon as possible. Cover or remove patient care equipment from construction area. Give way to essential patient traffic and interrupt dust producing activities to allow patients to pass. Seal holes, pipes conduits and punctures appropriately. Use a HEPA vacuum and/or damp disinfectant mop for cleanup of dust. Contact EMS when complete. Cover general construction waste before transport. The contractor shall restore all the affected areas by construction to their original state. All the utility work shall be performed by certified technician or workers. INFORMATION AND INFORMATION TECHNOLOGY SECURITY REQUIREMENTS 839.201 Contract clause for Information and Information Technology Security: The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract. VA INFORMATION AND INFORMATION SYSTEM SECURITY/PRIVACY LANGUAGE FOR INCLUSION INTO CONTRACTS, AS APPROPRIATE (Ref: 6500.6 Appendix C) GENERAL Contractors, contractor personnel, subcontractors, and subcontractor personnel shall be subject to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding information and information system security. ACCESS TO VA INFORMATION AND VA INFORMATION SYSTEMS A contractor/subcontrator shall request logical (technical) or physical access to VA information and VA information systems for their employees, subcontractors, and affiliates only to the extent necessary to perform the services specified in the contract, agreement, or task order. All contractors, subcontractors, and third-party servicers and associates working with VA information are subject to the same investigative requirements as those of VA appointees or employees who have access to the same types of information. The level and process of background security investigations for contractors must be in accordance with VA Directive and Handbook 0710, Personnel Suitability and Security Program. The Office for Operations, Security, and Preparedness is responsible for these policies and procedures. Contract personnel who require access to national security programs must have a valid security clearance. National Industrial Security Program (NISP) was established by Executive Order 12829 to ensure that cleared U.S. defense industry contract personnel safeguard the classified information in their possession while performing work on contracts, programs, bids, or research and development efforts. The Department of Veterans Affairs does not have a Memorandum of Agreement with Defense Security Service (DSS). Verification of a Security Clearance must be processed through the Special Security Officer located in the Planning and National Security Service within the Office of Operations, Security, and Preparedness. Custom software development and outsourced operations must be located in the U.S. to the maximum extent practical. If such services are proposed to be performed abroad and are not disallowed by other VA policy or mandates, the contractor/subcontractor must state where all non-U.S. services are provided and detail a security plan, deemed to be acceptable by VA, specifically to address mitigation of the resulting problems of communication, control, data protection, and so forth. Location within the U.S. may be an evaluation factor. The contractor or subcontractor must notify the Contracting Officer immediately when an employee working on a VA system or with access to VA information is reassigned or leaves the contractor or subcontractor s employ. The Contracting Officer must also be notified immediately by the contractor or subcontractor prior to an unfriendly termination. VA INFORMATION CUSTODIAL LANGUAGE Information made available to the contractor or subcontractor by VA for the performance or administration of this contract or information developed by the contractor/subcontractor in performance or administration of the contract shall be used only for those purposes and shall not be used in any other way without the prior written agreement of the VA. This clause expressly limits the contractor/subcontractor's rights to use data as described in Rights in Data - General, FAR 52.227-14(d). VA information should not be co-mingled, if possible, with any other data on the contractors/subcontractor s information systems or media storage systems to ensure VA requirements related to data protection and media sanitization can be met. If co-mingling must be allowed to meet the requirements of the business need, the contractor must ensure that VA s information is returned to the VA or destroyed in accordance with VA s sanitization requirements. VA reserves the right to conduct on-site inspections of contractor and subcontractor IT resources to ensure data security controls, separation of data and job duties, and destruction/media sanitization procedures compliant with VA directive requirements. Prior to termination or completion of this contract, contractor/subcontractor must not destroy information received from VA, or gathered/created by the contractor during the performance of this contract without prior written approval by the VA. Any data destruction done on behalf of VA by a contractor/subcontractor must be done in accordance with National Archives and Records Administration (NARA) requirements as outlined in VA Directive 6300, Records and Information Management and its Handbook 6300.1 Records Management Procedures, applicable VA Records Control Schedules, and VA Handbook 6500.1, Electronic Media Sanitization. Self-certification by the contractor that the data destruction requirements above have been met must be sent to the VA Contracting Officer within 30 days of termination of the contract. The contractor/subcontractor must receive, gather, store, back up, maintain, use, disclose and dispose of VA information only in compliance with the terms of the contract and applicable Federal and VA information confidentiality and security laws, regulations, and policies. If Federal or VA information confidentiality and security laws, regulations and policies become applicable to the VA information or information systems after execution of the contract, or if NIST issues or updates applicable FIPS or Special Publications (SP) after execution of this contract, the parties agree to negotiate in good faith to implement the information confidentiality and security laws, regulations, and policies in this contract. The contractor/subcontractor shall not make copies of VA information except as authorized and necessary to perform the terms of the agreement or to preserve electronic information stored on contractor/subcontractor electronic storage media for restoration in case any electronic equipment or data used by the contractor/subcontractor needs to be restored to an operating state. If copies are made for restoration purposes, after the restoration is complete, the copies must be appropriately destroyed. If VA determines that the contractor has violated any of the information confidentiality, privacy, and security provisions of the contract, it shall be sufficient grounds for VA to withhold OTHER REQUIREMENTS: Scheduling: Work shall be completed within normal business hours. Monday through Friday 8:00am 4:30pm, excluding Federal holidays. Interference with Hospital Functions: Contractor personnel are required to interrupt their work at any time so as not to interfere with hospital functions. This includes but not limited to, utility service, fire protection systems, and the passage of facility patients, personnel, equipment, and carts. Contractor Reporting: The Contractor Field Supervisor (CFS) will provide to the Contracting Officer (CO) an official company letter with a list of all personnel that will work on the project. The letter should provide the first name, last name and last 4 numbers of the social security number. The POC will make arrangement with the hospital police security section for the issuance of VA hospital identification cards. All contractors employees must wear the identification at all time while performing his/her duties on the installation. The identification must be visible in the torso area. Upon completion of the project all identification must be returned to the POC who will return them to the Police Service section. Each card not returned or lost will generate a payment to the Government on the amount of $25.00. The POC will coordinate parking and off load of supply and equipment arrangements with the police section and the Contractor. Status and Schedule of Work Reports: The contractor will do and submit on weekly basis daily logs to the COTR and the CO. The COTR reserves the right to change this statement or waive it depending on the complexity of the project and the contractor performance. Inspections: The daily work will be inspected by Contractor Field Supervisor (CFS) to be provided by contractor. This CFS will ensure that all the work is being performed according to the specifications and regulations involved in this contract. The COTR and VA Engineering Service personnel may regularly inspect the work and accept if satisfactory. 7.11 Safety: While in the performance of the work, the Contractors personnel shall abide by the safety requirements of the Medical Center. The Contractor shall exercise extreme care in always making the work area safe. The number one priority is to protect patients and hospital staff from any possible injury resulting from the equipment or materials. The Contractor is responsible for security of equipment each day. There is no storage area available on site. The hallways nor corridors shall not be used to store, pile or place construction materials or supplies. In the event of a situation requiring use of the corridor for such purposes, approval of the VA Medical Center Safety Manager/Officer shall be obtained previously. No painting material or supplies shall be placed where patients may recover them. Any safety violation or emergency situation should be reported to a hospital official immediately. The Contractor shall conform to all Federal, State, and local regulations governing painting chemicals and equipment, which may be in effect. All equipment used to perform the work will conform to OSHA regulations. 7.12. At the termination of work the POC shall assess the area before acceptance/activation. SUBMITTALS: 8.1.
