Network Contracting Office 8 (NCO 8) at Orlando VA Healthcare System is conducting market research to gain knowledge of potential qualified sources that can provide services for the Viera Outpatient Clinic (OPC) Lightning Protection System, located at Viera OPC, 2900 Veterans Way, Viera, FL 32940. Project scope: The Orlando VA s Outpatient Clinic in Viera Campus requires repairs, revisions, upgrades, and certifications. The contractor shall Contractor shall provide all parts, equipment, materials, and labor required to inspect VOPC antiquated lightning protection system at each of the structures located at the Viera OPC. They shall also hold the required lightning protection certification (UL Inspection Certificate) and be compliant with current electrical code compliance and UL 96A or NFPA 780. These certificates should carry a three-year expiration date to complement the NFPA three-year code review. Deliver a lightning protection system carrying an LPI-IP Inspection and Certification Program instead of the UL Master Label Inspection Certificate. For the purpose of this acquisition, the NAICS code is 811210 and the small business size standard of $34.0 is applicable. This survey is to determine the capability of potential sources and the availability of commercial sources and services pursuant to FAR Part 10. Capability statements shall include the following business information: a. UEI number, business name, and address. b. Point of Contact (Including name, title, addresses, telephone number, fax number and Email address) c. Business size for NAICS: 811210. d. Socio-economic status (e.g. SDVOSB, VOSB, 8a, HUBZone, WO, Small Business, etc.) Interested parties shall also, provide the following information: a. Provide a narrative that addresses your firm s ability to provide the services describe herein. b. Provide past performance for this type of service. c.
