The Government is not obligated, nor shall it pay for, any information received from potential sources as a result of this Sources Sought Notice. The West Palm Beach VA Health System is seeking business concerns capable of providing the West Palm Beach VA Medical Center Facility address listed below: West Palm Beach VA Medical Center 7305 N. Military Trail, West Palm Beach, FL 33410 The contractor shall furnish Professional Services to provide to the Government full service and repair on eighteen (18) Karl Storz 11272VHUK Cystoscopes, all Karl Storz factor trained service engineer labor with no exceptions, original equipment OEM Karl Storz parts, and certification in the form of technical documentation, original invoice, or service documentation that the quality of updates and upgrades is sufficient to guarantee continued performance and operation of the Cystoscopes listed in this contract to meet FDA and the original manufacturer Karl Storz factory standards. Contractor shall furnish a detailed field service report upon completion of work to Biomedical Staff in the Biomed Department. Payment will not be processed until a properly completed service report is received. The service report shall contain, at a minimum, the following information: Type, model and serial number (s) of all equipment on which maintenance was performed Total time spent performing maintenance Detailed narrative description of the services required Copies of all test reports Complete list of parts replaced and/or updates performed Date and time the repair was completed. Firms with interest and capability to include, Federal Schedule Contract Holders, Small Business, Veteran-Owned Small Business, Service-Disabled Veteran-Owned Small Business, HUB Zone Small Business, Small Disadvantaged Business, and Women-Owned Small Business Concerns and all other business types and sizes may respond by submitting a qualifications letter stating the following information: Name Capability Statement (to include the Counties/Locations of interest) DUNS # and SAM UEI Info Type of business (i.e., SDVOSB, VOSB, WOSB, etc.) Contact person-name-e-mail and telephone number Responses to this sources sought notice shall be sent via E-mail to be received no later than 4:00 PM EST- (1700) June 15, 2023.
