DEFINITIONS/ACRONYMS: (add as appropriate) Department of Veterans Affairs (VA) Contracting officer's representative (COR): A representative from the requiring activity appointed in writing by the CO to perform surveillance and to act as liaison to the contractor. II. GENERAL REQUIREMENTS: 1. Introduction: Clean and removal of sediments in a 2-million-gallon domestic water tank. This is to ensure the safety of VA Medical Center patients by providing safe and clean drinking water. 2. Place(s) of Performance: The Water Tank is located at the Lake Nona VA Medical Center Campus, 13800 Veterans Way, Orlando, FL 32827. 3. Background: The cleaning of the domestic water tank is to ensure safe and clean drinking water is provided to the VA Medical Campus and to ensure legionella growth is contained at a safe level. 4. Performance Period or Delivery Date: This service can be complete at any time as the domestic water tank can be isolated and will not interrupt water supplied to the Medical Campus. III. SPECIFIC REQUIREMENTS/TASKS AND ASSOCIATED DELIVERABLES Clean the 2-million-gallon domestic water tank and remove sediments. Tank will be cleaned without emptying water and shall remain in service for fire protection purposes. All equipment shall be sanitized prior to cleaning operations. Contractor shall submit his method of cleaning the water storage tank. Contractor shall provide a list of tools used to for cleaning the domestic water tank. Contractor shall also provide evidence of compliance with Florida Department of Environmental Protection (FDEP), Southwest Florida Water Management District (SWFMD), Occupational Safety and Health Administration (OSHA) and National Sanitation Foundation/American National Standards Institute (NSF/ANSI). Contractor will follow guidelines set forth in Occupational Safety and Health Administration (OSHA) for confined space, control of hazardous energy, and fall protection requirements. Contractor shall provide a visual inspection of the tank, including walls, roof, base and supply a written report of their assessment. Contractor shall be responsible for all pre and post disinfection testing requirements in accordance with AWWA C652-11 and state, local, and federal guidelines. These tests shall confirm that the water quality is appropriate for distribution, including, but not limited to pH, alkalinity, turbidity, odor, specific conductance, and chlorine residual. Samples shall be representative of the water quality in the tank. Contractor shall Provide a written assessment of the work Upon completion in no more than 14 calendar days of the completion date of the work. Contractor shall provide all equipment necessary to complete the work listed in this requirement to include equipment, materials, and testing needed to meet OSHA requirements for a safe work site. All materials and equipment shall meet the requirements of the Safe Drinking Water Act, AWWA C652-11, and NSF/ANSI Standards. Contractor shall ensure all fall protection procedures are followed in accordance with OSHA requirements. Government-furnished items and services: None IV. REPORTING REQUIREMENTS (if applicable) Upon arrival at the site please report to POC to gain access to the facility and entry into the water tank area. Provide a plan of what work is to be completed daily. V. CONTRACTOR EXPERIENCE REQUIREMENTS (if applicable) 1) General Certifications and/or Licenses required for cleaning of domestic water tanks and systems. (If applicable) 2) Provide POCs of where previous work of this nature has been performed.
